Accident Location: Sea and Land in West-19, Burhan
WHAT HAPPENED:
The IP was holding onto a lifting sub for a 6 ½” drill collar to guide it whilst it was being lifted from its basket by the off driller
side air winch. During the lift the IP lost his grip on the lifting sub, it slipped from his hand and swung towards the mast
leg. The IP attempted to stop it striking the leg and in doing so his left hand was crushed between the lifting sub and the
leg.
The IP was taken to the rig clinic for initial treatment and then transferred first to Adam Hospital and then later to Nizwa
Hospital where he was diagnosed as having fractured his left hand.
INJURY:

WHAT HAPPENED:
The IP was on the rig floor completing the nippling down of the blow-out preventer. After laying down the mousehole, the
crew had placed a an ill-fitting temporary cover over it. The IP stepped on the temporary cover which moved causing him to
slip, fall over and sustain an injury to his groin area.
First aid was administered by the rig medic and the injured man was sent directly to Salalah hospital for further treatment
on a deep laceration.

Accident Location: Dalma Energy, Nimr
WHAT HAPPENED:
The floor man (IP) & driller were inside the cellar nippling up the Tubing Head Spool (THS) onto the Casing Head Housing
assembly (CHH). Whilst attempting to align one of the bolts on the THS into the corresponding hole of the CHH assembly,
the IP placed his left hand inside the gap between the THS and CHH assembly. When the bolt had aligned the THS
dropped down onto the CHH and trapped IP’s left hand in between the two flanges.
The IP was immediately sent to the PDO Nimr clinic. First aid was given and then he was transferred to the Salalah
Hospital for further treatment. He was diagnosed with a deep cut wound at palmer side of the hand.

Accident Location: Salalah to Muscat road, Fatal motor vehicle incident
A fatal motor vehicle incident occurred on the Salalah to Muscat road on Friday, 24 January, 2014 at approximately 15:45
hrs. The deceased was a 41-year-old Pakistani truck driver working for Ajib Trading LLC, a sub-contractor to our hoist
contractor Shiv-vani.
WHAT HAPPENED:
The truck was transporting drill pipes from Shiv-vani’s hoist-21, operating in Saih Nihayda, to the Sino Gulf base in
Nizwa. En route the truck rolled over, which sadly resulted in the driver losing his life.
An investigation into the incident is underway.
Our thoughts and prayers are with the family, friends and colleagues of the deceased.

Accident Location: Al Noor MGI Phase 3A project ,Arabian Industries Projects,Nimr
WHAT HAPPENED:
The IP and an operator were working at height inside the bucket of a bucket truck. They were in the process of cutting
temporary wooden supports from a pipe rack and had just moved the bucket into position beneath one of the wooden
supports. While attempting to position themselves within the bucket to cut the support, the controls of the bucket were
accidently engaged. The bucket moved trapping the rigger between the guard rail of the bucket and the pipe rack
structure. The operator reacted quickly and managed to drop the basket both backwards and downwards to save further
injury. The PTW Holder who was at location raised the alarm and an ambulance arrived to evacuate the IP to the
construction camp clinic. He was later referred on to the PDO Marmul clinic and then on to the Salalah Hospital where he
was diagnosed as having fractured his ribs.

Accident Location: Desert Byrne Drilling, Lekhwair
WHAT HAPPENED:
During a rig move the truck was transporting a diesel tank secured on a flat bed trailer from Lekhwair to Yibal. Whilst
negotiating a sharp right hand bend he lost control causing the truck and trailer to roll over.
Both the driver and his passenger were initially taken to the Yibal PDO clinic for their initial treatment and then transferred
to Nizwa hospital for further examination and treatment where the driver was diagnosed with a fractured collar bone and
dislocated left shoulder joint. The passenger suffered only minor bruises and was discharged with painkillers.

Accident Location: Hoist 25 Nimr , Shaleem Petroleum
WHAT HAPPENED:
The IP was re-arranging a sewage pipe over the edge of the waste pit when he lost his balance and fell 1.5m down the
slope into the pit and in doing so twisted his right ankle. At 18:30 pm he informed the HSE Advisor from the camp site who
took him to the Sahara PAC Clinic where he received an injection and painkiller. On the 26th January the pain had not
subsided and so we was referred to the Badr Al Sama Hospital in Salalah for a specialist X-ray which revealed a fracture in
the right ankle.
INJURY:

Accident Location: Thulailat-151 well in the Marmul area, Vanguard
WHAT HAPPENED:
The incident occurred during the wireline logging operation. While the wireline operator was pulling the cable out of the
hole, the IP climbed on the deck of the unit to clean the winch area. After cleaning one side, he attempted to cross to the
other side between two wire drums. His foot became trapped and was crushed between the moving wire drum and a steel
frame. The operator immediately stopped the winch and reversed it to free the IP’s foot.
The IP was immediately taken to the Marmul PDO clinic for his initial treatment and then transferred to Salalah hospital
where he was diagnosed as suffering from multiple fractures of his right foot. He was later referred onto Khoula hospital,
Muscat for re-constructive surgery.

Accident Location: Ras Al Hamra project in Muscat, Al Turki
WHAT HAPPENED:
The IP was working on a balcony situated at roof height on one of the new director houses. Employees were protected
from a fall from height by barrier protection which had been fitted at the edge of the balcony. The IP lent against the barrier
which gave way and he and the barrier fell 10 metres down to the ground below. He was immediately given first aid and
then transferred to Khoula hospital where he was admitted to intensive care. He sustained multiple fractures.

Accident Location: Fahud, Midwesco workshop, Midwesco
WHAT HAPPENED:
A chief mechanic and a mechanic were removing a 18kg pin from a cross head and connecting rod for a JWS-400 pump
using a press. As the chief mechanic (IP) operated the press which pushed the pin vertically down out of the hole the other
mechanic crouched down and waited to catch the pin when it emerged fully from the hole. Unfortunately as the pin was
greasy, as it came out he lost his grip on it and it fell 30cm down onto the left foot of the IP who was still operating the
press. The crew administered first aid and then he was transferred to the Fahud PDO Clinic. He was then referred onto
Ibri hospital where he was diagnosed as suffering from a fracture to his foot.

Accident Location: Al Mukhtar, a subcontractor of the 4PL logistics contractor for PDO
WHAT HAPPENED:
The driver arrived at Rig 7’s new location with a full load of pipes and casing joints. After he had unstrapped the load, the
Truckoman rig moving crew began to unload it using a forklift truck. Whilst the unloading was taking place the driver at the
back of the trailer attempted to remove wooden dunnage which separates different levels of the load and allows the forks
of the forklift to slide under the piping. In the process of removing the dunnage he caused a pipe to roll which trapped his
hand between the pipe and a trailer support stanchion. He managed to remove his hand but not before the crush had
injured two fingers of his right hand. The driver was taken to the Marmul PAC clinic where the doctor treated him and then
referred him to Salalah hospital for X Rays which diagnosed him as having a fracture of the middle finger.

WHAT HAPPENED:
Whilst securing tubular pipes on a trailer at MB completion workshop in Fahud, the IP lost his balance and fell from the
trailer bed (1.8 m height). He was immediately transferred to PDO clinic for first aid, after which he was transferred to
Nizwa Hospital for further treatment. X-rays showed forearm fracture and dislocation of the right elbow joint. The IP was
discharged with a cast on the right elbow.

Accident Location: Lekhwair, Oman Oil Marketing sub contractor of KCA Deutag.
WHAT HAPPENED:
The (IP) and his colleague (the driver, unhurt) were in their way back to Muscat after they have delivered engine oil drums
to PDO Rig-11. The driver faced some sand dunes on the road where he was not able to see them from far due to windy
and bad weather. The truck slipped out of the road when drove over the dunes and lost control. Police rescued them and
took them to PDO Lekhwair clinic and later on both of them were sent by the ambulance to Ibri hospital. The driver was
discharged after the medical examination but the helper suffered fracture of L1 vertebra and canal compromise due to
fragments into the spinal canal.

Accident Location: SAIH RAWL, OFSAT .
WHAT HAPPENED:
An Ofsat oil field truck was approaching a left hand turn to Rig 85 which was based just off the Saih Rawl to Barik graded
road. A diesel tanker had travelled along the road in the opposite direction and had left pockets of dust lingering in the air
along the road. The junction to the rig was shrouded in dust and as the Ofsat truck turned left at the junction a Galfar 3
tonne canter approaching from the opposite direction collided with the truck which was across his lane.
The canter driver and its two passengers were taken initially to the Galfar clinic and then to the Qarn Alam PAC Clinic
before being transferred to Nizwa Hospital where X-rays revealed that the driver had suffered multiple fractures to both his
legs. He had also suffered a strain to his left lower chest and his left elbow. One of the passengers also sustained
abrasions on his neck and left knee. The second passenger of the canter and the Ofsat driver escaped without injuries.

Accident Location: RIMA SMALL FIELDS, ABRAJ ENERGY.
WHAT HAPPENED:
After completing maintenance of the rig’s mud pump the crew turned it back on. The Derrick Man who was part of the
mechanical crew noticed an abnormal noise coming from the pump and to identify the source of the noise he walked
around the pump checking it. As he walked up to the pump’s external rotating crankshaft he was struck by a rotating metal
bar which had inadvertently been left connected to the crankshaft. The bar had been used to manually rotate the
crankshaft when it had no power and the crew had forgot to remove it after they had finished. He was knocked
unconscious and was found on the ground by the rig crew.
He was taken to the rig clinic for initial treatment and then transferred to Haima Hospital where he was diagnosed as
having a fractured right leg and injuries to his left hand. The doctor referred him on to Nizwa Hospital for further treatment.

Accident Location: Yibal, MB Petroleum Services .
WHAT HAPPENED:
After rigging down the Wireline equipment, the Wireline tools were being loaded onto a Hiab truck. During the loading
process, the truck driver noticed that a 12” well head crossover had incorrectly been placed vertically on the back of the
truck. He attempted to push the crossover over into a horizontal position with his left foot whilst his right foot was
positioned on a lubricator. Despite holding the side of the truck for balance, as he pushed the cross over his right foot
slipped off the lubricator, causing him to fall over and his left wrist struck another lubricator on the truck.
He was immediately taken to PDO Yibal clinic for first aid treatment and then transferred to Ibri Hospital for further
treatment where X-rays revealed he had fractured his left wrist.

Accident Location: Nimr, Dalma Energy Rig 47 .
WHAT HAPPENED:
During a rig up operation the crane operator needed to leave the crane operating cabin towards driving cabin. To do this he
had to first step down onto the floor of the crane before climbing down the fixed ladders attached to the side of the crane to
the ground. Whilst descending from the cab his left foot slipped and he did not have sufficient grip on the handholds to
prevent his loss of balance. He fell 1.3 meters down to the ground, landing heavily on his left hand.
He was taken immediately to the rig camp clinic for treatment and after consultation with the Marmul PDO clinic doctor he
was transferred to Salalah hospital where X-rays revealed he had fractured both his left wrist and forearm.

Accident Location: Bahja, Dalma Energy Rig-58 .
WHAT HAPPENED:
The Tool Pusher was on the rig floor in preparation for circulation and pulling out of hole. Whilst on the rig floor he needed
to go and check the shakers but as he walked past the rotary table his footing slipped, his ankle twisted and he fell to the
ground on the rig floor.
He was immediately taken to the rig camp clinic for treatment by the medic and then transferred to Haima Hospital where
X-rays revealed he had fractured his right lower leg.

Accident Location: Nimr NRPS, Wood Group CCC.
WHAT HAPPENED:
The IP and his helper were carrying out hot bolting on the fire water tank T4808 pipe line flanges at NRPS as pre-shutdown
activity. The IP was initially hammering the impact spanner to loosen nuts while the helper was holding the combination
spanner on the lower nut. The IP got tired and asked the helper to do the hammering (switching roles). Whilst the helper
was hammering to loosen the bolts, the IP observed that the impact spanner was getting loose and instinctively attempted
to put it back without communicating with the helper to stop hammering. The IP got his right index finger hit by the hammer
and he was immediately taken to Tawoos clinic. After consultation with Marmul PDO clinic doctor, the IP was transferred to
Haima hospital where X-rays revealed fracture of distal phalanx right index finger.

Accident Location: Rima, Weatherford.
WHAT HAPPENED:
The incident occurred when a joint of drill pipe with a scraper attached to it was lifted with the forklift, using a single
webbing sling. The IP was guiding this load to be placed on supports on the ground when he stepped backwards and
tripped over another joint of pipe. When he fell he tried to support himself on the sling which then came off the fork and the
joint with the scraper fell on his left foot.
He was immediately taken to the STS Amal clinic for first aid, and then transferred to Salalah Hospital where X-rays
revealed he had fractured both bones in his left leg.

Accident Location: Lekhwair, Al Bunyaan subcontractor for EMC.
WHAT HAPPENED:
The technician was repairing one of the A/C chiller units in the Lekhwair Pumping Station at substation 10 and in doing so
he needed to check the belts tension. While doing that his left hand fingers got trapped between the belts and the
pulley. This caused injury on three fingers of his left hand.
He was immediately taken to Lekhwair PAC clinic for first aid, and then escorted to Ibri Hospital where diagnosed as
Complete amputation of the distal phalanx of the left little finger. Partial amputation of the distal phalanx of the ring finger.
He was taken by his company to Khoula hospital and then he discharged himself to continue treatment back in India.

Accident Location: Yibal B station
WHAT HAPPENED:
During the regular maintenance the team believed that they had isolated and depressurised the launcher down to
atmospheric pressure down from the 56 bar in the gas line. They then attempted to open the ring lock trap door at the end
of the pig launcher but as they did so the door violently flung open. It swung right to left on its hinges striking the supervisor
on the arms and then sheared off coming to rest 2 metres from the launcher. Two other members of the team received
minor injuries as a result.
The emergency response protocol was initiated, Yibal B was shut down and fire and ambulance services were mobilized to
the site. The LECC was also activated. The three injured people were transported to the PDO clinic for treatment and
stabilization before being transferred to Ibri Hospital for further checks and scans. The most severely injured person was
further transferred to Khoula hospital where he was on operated on for a multiple fracture to his left arm and fracture to his
right wrist. The other injured people sustained bruises and minor cuts to the face & chest.

Accident Location: RAH school project, MAF.
WHAT HAPPENED:
At around 7:30 hrs the helper was walking across the new school building site and was carrying electrical tools in both
hands. As he walked he lost his balance on a layer of loose sand which had covered a pavement. He fell down and as he
was carrying tools he was unable to cushion his fall causing him to fall heavily on his left elbow.
He was given first aid treatment immediately by the foreman and a HSE supervisor and transferred to the Apollo hospital in
Muscat. In hospital he received surgery for his injury and remained an in-patient for 3 days.
It is not yet established how long the helper will be absent from work as the contractor failed to notify PDO of the incident
or injury. Appropriate action has been taken against the contracting company and all the individuals responsible or
involved in the failure to notify the incident.
An investigation is underway.

Accident Location: Carillion’s coastal yard , MAF.
WHAT HAPPENED:
Whilst the helper was helping offload steel pipes from the rear of a lorry in the Carillion’s coastal yard, his attention was
distracted by a colleague. As he was not concentrating on his task he was unable to react when another colleague on the
back of the lorry moved the pipes backwards trapping his thumb between the tail gate and a pipe.
He was immediately taken to Khoula hospital where they diagnosed a serious injury which included a distal amputation and
fracture of his right thumb. He was admitted to hospital and discharged 5 days later.
He is expected to be off work for six weeks.

WHAT HAPPENED:
The helper, working with a team of plumbers in the school administration block was cutting a groove into a wall with a
grinder when the glove he was wearing snagged on some wiring. This led to him cutting the back of his hand with the
grinder he was using.
He was immediately given first aid and then transferred to the Apollo Hospital for minor surgery.
He is expected to be off work for 42 days.

WHAT HAPPENED:
The technician was performing a Preventative Maintenance work on Gas cooler fan motor, his index finger got trapped
between motor pulley and belt causing deep lacerated wound in the right little finger.
He was immediately taken to Qarn Alam PDO clinic where he received initial treatment and later referred to Nizwa hospital
for further management.
At Nizwa hospital, X-Ray revealed fracture of the tip of the finger, dressing was applied and injured person discharged on
antibiotic and analgesic medications, he will be seen by Nizwa doctor after three days for further assessment.
He is expected to be off work for 5 days.

WHAT HAPPENED:
Whilst descending the stairs from the tea room cabin he slipped and his grip on the handrails was insufficient to prevent
him losing his balance and falling off the stairs an down to the ground landing heavily on his left hand and knee.
He was immediately taken to the SMS-4 clinic for treatment by the medic and arrangements were made to transport him to
the hospital at first light. At the hospital X-rays revealed he had fractured his wrist and bruised his knee.

Accident Location: Road from Nimr towards Rima, Nimr.
A triple fatality motor vehicle incident occurred on the road from Nimr roundabout towards Rima on Tuesday, 1st July, 2014
at approximately 11:00 hrs. The deceased were 42-year-old Omani canter driver, 27-year-old Indian Heavy Equipment
Mechanic and 22-year-old Indian Mechanic all working for Al Turki Enterprise.
WHAT HAPPENED:
A canter carrying Al Turki employees travelling from Runib a 30km from Nimr roundabout towards Rima had a head on
collision with a 4PL OTO Trailer registered in the name of Desert Wall Trading. The driver and two passengers in the canter
passed away and the driver of the trailer had fractured his right leg and medvic to Salalah Hospital.
An investigation into the incident is underway.
Our thoughts and prayers are with the family, friends and colleagues of the deceased.

Accident Location: Amal 144 , Nimr area.
WHAT HAPPENED:
The completion with a down hole pump was pulled out of the hole and laid out on a pipe rack just outside of the location
together with some 4½” tubing joints. The Driller rolled the pump assembly over the rack to have a better look at the pump
unit which caused a joint of 4½” tubing to roll to the edge of the pipe rack. Since the stoppers at the end of the rack were
not in place, the 4½” tubing fell from the edge of the pipe rack onto the supervisor’s left foot.
He was initially seen by STS Amal clinic and then referred to Haima Hospital where X-Ray revealed a fracture of 2nd toe of
the left foot.
He is expected to be off work for 6 weeks.

Accident Location: Sea & Land Rig #8, Yibal area.
WHAT HAPPENED:
The Mechanic was attempting to open the radiator cap of a generator engine to check the water level, the cap was too tight
due to water pressure and temperature. Whilst he tried to loosen the cap using a spanner , the cap open forcefully flushing
the hot water on his left hand, left leg and side of his face.
He was initially seen by the Rig-8 medic where he was given first aid and then transferred to Ibri Hospital, later he
requested to be transferred to Rustaq Hospital. The doctor in Rustaq Hospital decided to transfer him to Khoula Hospital
for treatment of second degree burns.
He is expected to be off work for at least 2 weeks.

Accident Location: Muscat – Salalah Highway.
WHAT HAPPENED:
The driver was in his way to Nimr from Muscat driving an articulated trailer loaded with chemicals when The prime mover
rolled over off to the left side of the carriageway, clear of the road and come to rest upside down, after experiencing a tyre
blowout on the prime movers left side rear tyre. The driver managed to get out of the vehicle before its burst into flames for
unknown reasons.
He was taken by passerby to Haima Hospital where he was stabilized. Then he was referred to Khawlah Hospital where XRay revealed a fracture of left elbow.
He is expected to be off work for at least 6 weeks.
An investigation into the incident is underway.

Accident Location: Al Huwaisah, RIG45.
WHAT HAPPENED:
The roustabout and the mud tester were installing a fly pump in the cellar which was suspended by webbing from the forks
of a forklift whilst it was being fixed in position. Whilst the roustabout was attempting to bring the fly pump into position it
swung across towards the BOP and his right hand ring finger became crushed between the BOP and fly pump.
The incident resulted in a deep cut and fracture to his finger which had to be reattached in Muscat Private hospital after he
had first received treatment in the rig clinic.
He is expected to be off work for at least 4-6 weeks.
An investigation into the incident is underway.

WHAT HAPPENED:
A mechanical crew was in process of removing scrap pipes from a construction site in Natih, using a trailer & a JCB. The
banksman was giving signals to the JCB operator and whilst he stepped backward he slipped on uneven ground and fell on
his knees.
He was initially seen by Fahud Renaissance PAC clinic and then referred onto Ibri hospital where X-rays revealed a
fracture of his left knee cap.
He is expected to be off work for 4 weeks.

WHAT HAPPENED:
The Floorman was making pipe X-over connections. Whilst he was attempting to connect the next section of pipe which
was suspended by the winch, without warning the winch jerked resulting in a closing of the gap between the X-over
and pipe and in doing so it trapped two of the Floorman fingers on his right hand.
He was provided initial treatment in the PAC and then transferred to Thumrait hospital for further treatment after being
diagnosed of fractures in his third finger.
He is expected to be off work for at least 2 weeks.

Accident Location: Midwesco Hoist 11, Yibal.
WHAT HAPPENED:
Whilst the Hoist was laying down drill pipe from the rig floor to the catwalk, one of the pipes drifted out from the V-door.
Derrickman attempted to place it back onto the V-door and asked the driller to lift it by using the winch line. In same time;
Derrickman was aligning the pipe by holding it at one end. The pipe however swung forward crushing his hand against the
substructure column.
Initially, the derrickman was taken to Yibal PDO Clinic for first aid, after which he was referred to Ibri Hospital for further
treatment. X-rays revealed he had suffered a deep cut to his middle & ring fingers and fractured his little finger.
He is expected to be off work for at least 10 days.

Accident Location: Ba Omar Oil services- Hoist 55, Marmul.
WHAT HAPPENED:
The derrickman was asked to release the tubing hanger. To do so he had to undo 6 tie bolts and as he was releasing the last bolt there
was a sound of escaping gas from the flange. This scared him and he jumped into the cellar to escape. As he landed he injured his left
hand which struck equipment in the cellar.
He was initially seen by the PAC clinic where he was given first aid and then transferred to Sultan Qaboos Hospital in Salalah; there Xrays revealed a displaced fracture of his left hand thumb & index finger. Afterwards he was transferred to Khoula hospital for further
treatment.
He is expected to be off work for approximately 4 weeks.

Accident Location: WGCC MVI Nimr Amin.
WHAT HAPPENED:
The driver was driving a canter lorry transporting four WGCCC workers from Nimr to their worksite in Amin. As he approached the
Amin B junction he maneuvered to turn left off the main Nimr/Marmul road but as he started to cross the opposing carriageway a
private vehicle travelling in the same direction attempted to over take him. Realizing too late that the lorry was turning, the third
party driver attempted to brake but lost control and struck the canter at an angle on its rear tailboard. The impact was so severe it
caused the canter to tip over onto its side.
Both drivers were immediately taken to the PDO clinic Nimr by ambulance. The canter driver was transferred to Haima hospital where
he was diagnosed as having fractured his metatarsal bone in his left foot and was given a 3 day sick leave. He was further referred
onto Sohar hospital for specialist treatment (report awaited).
The third party driver had no injuries which required medical attention
He is expected to be off work for 5 weeks.

Accident Location: Dalma Energy Rig-47 Nimr
WHAT HAPPENED:
During rigging up, the crew were in the process of fixing the mud boat to the substructure using two cranes. The two
structures are locked in place by inserting connecting pins through holes in each structure which have to be aligned.
As the Night Toolpusher was attempting to align the holes to insert the pin, the substructure moved slightly which caused
the pin to move sidewise trapping the middle finger on his left hand between the pin and the substructure.
He was initially seen by the Rig medic, where he was given first aid and then transferred to Sultan Qaboos Hospital in
Salalah where they diagnosed an amputated tip from the base of the nail onwards.
He is expected to be off work for approximately 3 weeks.

WHAT HAPPENED:
The operator was driving his vibrator down a steep slope with rough terrain when his left leg struck against the vehicles
dashboard, causing pain in his left knee. He immediately stopped and received first aid treatment at the site before being
transferred to the Government hospital in Adam. He was further transferred to Nizwa Hospital where x-rays confirmed a
fracture of the left thigh bone. He was again transferred to Khoula Hospital in Muscat where he underwent an operation to
set the fracture.
He is expected to be off for work for several weeks

WHAT HAPPENED:
Whilst the mason was redirecting the air flow from a rotating pedestal fan he was distracted by his colleague and as he lost
his attention his left thumb slipped through the protective wire mess of the fan and was struck by the fans blade.
The impact amputated the tip of his thumb which included his nail. He was immediately taken to PDO MAF Clinic for first
aid and later referred to Khoula Hospital where they treated his injury. He was admitted to hospital and discharged 2 days
later.

Accident Location: RAH project, MAF, ATE in-site workshop
WHAT HAPPENED:
The welder had just drilled a hole through a steel angled bracket using a bench pillar drill and he then raised the drill to
release it from the bracket. The bracket though stayed attached to the drill bit and continued to spin as the drill powered
down. Instead of waiting for it to stop spinning, he attempted to remove the bracket by grabbing it and in doing so his left
hand glove became entangled around the drill bit. The tremendous force cause the end of his ring finger to be amputated.

WHAT HAPPENED:
After finishing a rig move of the Dalma 72 rig, two crane operators were in process of securing the crane hook to the front
of the crane for safe transit to the next location. To secure it, one of the operators swung the hook towards the sling
attached to the front of the crane. The other operator attempted to loop the sling over the hook but whilst doing so his
middle finger on his left hand became trapped between the sling and the hook as it started to swing back. The pressure
caused a fracture and a cut to his finger.

WHAT HAPPENED:
Whilst preparing for cementing, the rig crew needed to fit a 5” drill pipe pup joint by utilising the mouse-hole. The floorman
installed a cross over, circular head and low torque valve on the top of the pup joint and then connected the cir hose to the
valve. He didn’t though connect the winch line to the circular head. As the pup joint was lifted up out from the mouse hole
the pup joint flipped over due to the weight exerted on it by the hose causing the low torque valve to flip back down and hit
the floorman on the right leg just above the ankle.

WHAT HAPPENED:
Whilst the crew were running drill pipe in the hole the roustabout held a slip by its handle in order to set the drill pipe in the
hole. When the driller lowered the drill pipe and elevator the body of the elevator dropped down onto the handle of the slip
and trapped the roustabout’s two right hand fingers holding the slip handle.
He was immediately taken to the rig camp clinic for first aid and then referred to Haima hospital where X-rays revealed a
fracture on the tip of the middle & ring finger.

WHAT HAPPENED:
Prior to running in the hole with 13-3/8” surface casing, the crew were in the process of opening the stabbing board’s
working platform into its working position when it became separated from the stabbing board frame and fell 8.7 metres
down to the rig floor where it struck the Assistant Driller on the right side of his head & his shoulder.
The crew immediately attended to him and then he was transported directly to Nizwa Hospital for assessment &
treatment. In the hospital, X-rays & CT scans revealed serious injuries to his spinal column, broken ribs and a large
laceration to his head. He is currently in a critical condition.

WHAT HAPPENED:
Whilst the derrickman, using a pipe wrench was tightening connections on the stuffing box for the beam pump the pipe
wrench slipped off the stuffing box causing him to lose his balance and he fell down on the ground landing heavily on his
left hand on the concrete platform.
He was initially given first aid at the site, however the next day he experienced further pain in his left elbow and so he was
sent to see the doctor in the PDO Marmul clinic for a follow up investigation. He was referred to Salalah Hospital where an
X-ray revealed he has fractured his left arm at the elbow.

Accident Location: OFSAT Casing Yard, Fahud.
WHAT HAPPENED:
The crew were offloading casing joints and had just placed four of them on the pipe rack. They then removed the lifting
hooks from the ends of the casing and the crane operator raised the boom ready to swing across to the truck for the next
lift. As the boom lifted one of hooks from a sling snagged on the underside of a length of wooden dunnage on the casing
stack which dislodged it, resulting in one of the casing joints rolling across the stack. The crew member was standing
directly in the path of the casing joint and it struck him on his right leg below the knee.
He was initially examined and treated in the Sahara PAC clinic at Fahud then refereed to Ibri hospital where an X-ray
revealed a fracture in the right leg.

WHAT HAPPENED:
The derrickman was standing on top of the pipe ram bonnet of the rig’s Blow Out Preventer (BOP) in order to see and
clean the bell nipple groove situated above the BOP. The ram bonnet is not designed for access to height so he was
wearing fall arrest equipment whilst he held a pressure wash gun in his right hand and the annular Preventer with his left
hand. As he operated the gun the back pressure caused him to lose his balance and the guns high pressure water jet
struck his left hand causing a cut to his wrist.
He was initially taken to the rig clinic for first aid and then transferred to the PDO Qarn Alam clinic then on to Nizwa
hospital for further treatment.

WHAT HAPPENED:
He was stacking curbing stones on a wooden pallet and whilst doing so he placed his left foot upon an upright curb stone
which was already on the pallet. He became distracted and his foot moved and dislodged an adjacent curb stone on the
edge of the pallet which then fell over landing on his right foot.
He was provided with first aid and then transferred to hospital for investigation and treatment where they diagnosed that he
had suffered a fracture to his metatarsal bone in his foot.

Accident Location: Shaleem Base,WPH-68,Nimr.
WHAT HAPPENED:
Whilst attempting to correct an alignment issue with the travelling block on a hoist which was being commissioned, the
crew were re-spooling the drill line onto the draw works drum. To maintain control of the drill line during the spooling, the
drill line was secured to the sand line via a snake line which ensured that after the last wrap of the drill line was removed
from the travelling block it would not fall. As the last wrap of the drill line spooled from the travelling block the snake line
failed which caused 20 meters of 1-1/8” drill line to fall to the ground and some hit the floorman on the left arm.

The Roustabout was pulling out and laying down a 4 1/2” tubing. He was opening the tubing elevator, when it swung open

was diagnosed with partial amputation of the tip of the finger.

WHAT HAPPENED:
While the crew was making up the bottom hole assembly on the rig floor, the night tool pusher and the floorman were
handling the bit breaker to place it into the slot of the rotary table, the roustabout came behind the tool pusher for assistant
and he garbed the breaker and when they got it into the hole the breaker was heavy enough to trap the roustabout finger
on the rotary table.
He immediately received first aid on the rig site and then was taken to rig camp clinic for further treatment. Later, he was
referred to Adam hospital and the x-ray revealed a fracture of the left middle finger with bone fragments.

WHAT HAPPENED:
While running in hole of 5" drilling pipe, the Derrick man was using his left hand to remove the slips and he put his right
hand on one of the tong Jaws which was in open position. In the process of lifting the slips, he inadvertently pressed the
jaw, causing the tong jaws to close and pinch his right hand thumb.
He immediately received first aid at the camp clinic by the rig medic, and then he was transferred to PDO Marmul clinic for
further treatment. Later, he was referred to Thumrait hospital where the x-ray revealed a fractured right hand thumb.

Accident Location: Burhan,Rig-69,Qarn alam.
WHAT HAPPENED:
While in the process of lifting drilling tools from ground to Rig floor using crane, the crane operator noticed that the crane
hook block was stuck in the boom sheaves and he took action to free the stuck line by holding the line with his hand and
shaking it. Suddenly the line got free and the hook block came down due to its weight pulling in his right hand into the block
and the line causing crush injury.
He received initial first aid treatment by Rig medic diagnosed with complete loss of the ring finger tip and damaged skin
and tissue of little finger tip. In consultation with PDO Fahud clinic, he was referred to Nizwa Hospital for further treatment.
