THT is working on other grants to support ongoing costs. THT is considering the role of an Accountable Care Organization. What is an Accountable Care Organization? An Accountable Care Organization (ACO): Integrated health care system Primary care physicians, specialists and hospitals work together to manage the overall care of their patients This done in such way that there is substantial cost savings Savings are then shared amongst the partners ACOs continued . . . . The ACO concept is a critical piece of the government’s current health reform agenda. ACOs aim to create a health system focused on coordinated care and clinical best practices by holding all members of a patient’s care team jointly responsible for the quality and cost of care. How do we get ready for ACO reimbursement? We must build an infrastructure that includes: 1. Health Information Exchange 2. Improvement in key quality health outcomes 3. Improve our inefficient health care workflow and processes 4. Demonstrate that we can reduce health care cost 5. Collaborate with area health care providers and institutions (this is a requirement to be a part of an ACO)
