To be eligible for the GUIDE program, people must meet the
following:

Have Alzheimer’s disease or another type of dementia as

Is enrolled in Medicare Parts A and B
n
Is not enrolled in Medicare Advantage or another Medicare
health plan, including Special Needs Plans (SNPS) and the
Program of All-Inclusive Care for the Elderly (PACE)
n
Has Medicare as their primary payer
n
Not enrolled in Medicare hospice benefit
n
Is not a long-term nursing home resident
n
Is not aligned to another GUIDE model program
