BSEE 4 – ELECTIVE 1 ELECTRICAL BUILDING AUDIT LOCATION: AUDIT DATE: AUDIT PERFORMED BY: AUDIT TEAM: WESTERN MINDANAO STATE UNIVERSITY - COLLEGE OF HOME ECONOMICS AUGUST – SEPTEMBER 2025 EE ELECTIVE 1 - GROUP 2 Darayan, Ansar Castillon, Eran Escandar, Rafiq Limen, Christian Zaragoza, Casey FIRST FLOOR AUDIT FINDINGS NO: 1-A CATEGORY: SERVICE DROP RISK LEVEL: HIGH FINDING: UNSAFE SERVICE ENTRANCE The service entrance doesn’t have any weathering protection. It has exposed line service conductors. The service entrance is poorly supported by insulators on wood instead of rigid masts. Open Splices are visible, and the insulation looks aged. VIOLATIONS: PEC 2.30 (Services) – Requires proper service head, conductor support, and drip loops. PEC 3.14 (Splices and Terminations) – All splices must be made with approved devices and contained in junction boxes. PEC 1.10 (Guarding of Live Parts) – Live conductors must be enclosed/guarded from accidental contact. OSHS Rule 1210 – Requires electrical installations to conform to PEC for worker/public safety. RECOMMENDATIONS: Replace old insulator mounted service entrance with a proper service mast and weather head Relocate service conductors away from combustible materials and enclose all splices in proper junction boxes and connectors. 1st Floor - Outdoors FINDINGS NO: 1-B CATEGORY: WIRING METHODS AND PROTECTION RISK LEVEL: MEDIUM FINDING: The air conditioning unit is wall-mounted, but above it is an open ceiling cavity with exposed conductors. Wires enter the cavity from an open hole and appear to be unsupported. There is a conduit (right side) that is improperly terminated or not securely fastened. VIOLATIONS: UNSUPPORTED CONDUCTORS NEC 300.11(A) – "Raceways and cables shall be securely fastened in place and supported." EXPOSED WIRING AND OPEN BOX NEC 300.15 – "Where cable or raceway enters a box, fitting, or enclosure, it shall be made with a fitting designed for the purpose." IMPROPER PENETRATION NEC 300.4(A)(1) – "Cables passing through holes in wood framing members shall be protected against physical damage." IMPROPER CONDUIT TERMINATION NEC 300.10 – "Raceways shall be continuous and securely fastened in place." RECOMMENDATIONS: Install a weatherproof or sealed junction box where cables enter the ceiling. All conductors should be routed through complete conduits or raceways with approved connectors and bushings. Close off the open ceiling cavity with a solid, noncombustible cover panel. If this is outdoor exposure, ensure all equipment and boxes are NEMA 3R or IP65 rated. 1st Floor - Outdoors FINDINGS NO: 1-C CATEGORY: WIRING METHODS AND PROTECTION RISK LEVEL: HIGH FINDING: EXPOSED ELECTRICAL SPLICING OUTDOORS Instead of proper junction boxes, the electrical wiring seems to be only joined and wrapped with electrical tape and may cause short circuits or become fire hazards. The wiring appears to be unsupported and the red conduit seen in the picture is not properly terminated or sealed. Open-ended conduits allow moisture and dust to enter, which can deteriorate insulation. VIOLATIONS: EXPOSED SPLICES AND IMPROPER TAPING PEC Part 1, Sec. 3.10.9.2 – All splices shall be made with approved splicing devices and enclosed in junction boxes. UNSUPPORTED WIRING PEC Part 1, Sec. 3.00.8.1 – Cables and conductors must be secured and supported at proper intervals. IMPROPER USE OF CONDUIT PEC Part 1, Sec. 3.10.4.5 – Conduits must be properly terminated with fittings, bushings, and covers to protect conductors. RECOMMENDATIONS: Properly enclose conduits and secure electrical splicing 1st Floor Outdoor Path FINDINGS NO: 1-D CATEGORY: CIRCUIT BREAKER- Maintenance and Workmanship RISK LEVEL: HIGH FINDING: EXPOSED AND UNMAINTAINED LIVE WIRES Wires are stripped and exposed, which is an electrical hazard. They are also observed to be old, brittle, and dusty, which suggests it is clearly unmaintained. TANGLED WIRING Wires are tangled and hanging loosely from the ceiling. CIRCUIT BREAKER INACCESSIBILITY The circuit breaker is located right under the roof, out of reach. IMPROPER WIRING CONNECTIONS Multiple wires are connected without the proper use of junction boxes and connectors. VIOLATIONS: WIRING VIOLATION PEC Part 1, Sec. 1.10.1.3 – Electrical installations must be in neat and workmanlike manner. PEC Part 1, Sec. 1.10.1.11 – Live parts of electrical equipment must be guarded against accidental contact. IMPROPER SPLICING / CONNECTIONS OUTSIDE JUNCTION BOXES PEC Part 1, Sec. 3.10.9.2 – All splices must be contained in approved boxes with covers. BURNT/DAMAGED INSULATION PEC Part 1, Sec. 3.10.2.2 – Conductors with damaged insulation shall not be used. PEC Part 1, Sec. 2.10.1.5 – Service drops and service entrance conductors must be securely installed and protected from damage. OSHS Rule 1210 – All electrical wiring and equipment must be maintained in a safe and serviceable condition. RECOMMENDATIONS: If Wiring and Circuit Breaker are inactive and no longer operational, terminate connections and remove or replace wiring connections. Proper splicing and connection practices should be followed. Secure wiring in junction boxes and connectors. 1st Floor - Outdoors FINDINGS NO: 1-E CATEGORY: OUTLET AND SWITCH INSTALLATION RISK LEVEL: HIGH FINDING: MISSING COVER The electrical outlet doesn’t have a cover. VIOLATIONS: MISSING ELECTRICAL COVER PEC Part 1, Sec. 3.10.6.1 (Boxes and Fittings – Covers) - In completed installations, each box shall be provided with a cover, faceplate, or fixture canopy. OSHS RULE 1210, SEC. 1214.01 (INSTALLATION AND MAINTENANCE) - Electrical equipment shall be maintained in a safe condition to prevent danger from accidental contact with live parts. RECOMMENDATIONS: Secure all electrical outlets with proper covers, face plates and fixed canopies. 1st Floor – Oven Room FINDINGS NO: 1-G CATEGORY: WIRING METHODS AND PROTECTION RISK LEVEL: HIGH FINDING: One junction box is missing a cover. Cables are organized but may be densely packed. VIOLATIONS: MISSING JUNCTION BOX COVER NEC Article 314.25 – Covers and Canopies: In completed installations, each box shall have a cover, faceplate, lamp holder, or luminaire canopy. POTENTIAL BOX OVERFILL NEC Article 314.16 – Number of Conductors in Outlet, Device, and Junction Boxes: Boxes shall be of sufficient size to provide free space for all enclosed conductors. CONDUIT ROUTING AND SUPPORT NEC Article 352.30 – PVC Conduit Support: PVC conduit shall be supported at intervals not exceeding 3 ft (0.91 m) and within 3 ft of each outlet box. RECOMMENDATIONS: Install a proper cover on the junction box immediately to prevent contact with live conductors. Verify box fill calculation per NEC Table 314.16(B) to ensure it's not overcrowded. Label the conduit or install a circuit directory for better system management. Ensure all conduit runs are securely supported as per NEC standards. 1st Floor – NEAR ENTRANCE FINDINGS NO: 1-H CATEGORY: WIRING METHODS AND PROTECTION RISK LEVEL: HIGH FINDING: MISMATCHED JUNCTION BOX AND JUNCTION BOX COVER Orange cover is partially present but not secured. Exposed conductors are hanging outside the box. Electrical tape used to wrap or bind conductors. PVC conduit entries are not properly sealed or terminated. VIOLATIONS: NEC 314.25 – Covers and Canopies - In completed installations, each box shall have a cover, faceplate, lampholder, or luminaire canopy that is suitable for the use. Covers shall be of the same shape as the box and designed to provide complete closure. EXPOSED LIVE CONDUCTORS NEC Article 300.15 - Boxes, Conduit Bodies, or Fittings - Where the conductors are not visible and accessible after installation, they shall be installed in an approved box or fitting. NEC Article 110.12(A) - Electrical equipment shall be installed in a neat and workmanlike manner. RECOMMENDATIONS: Replace the cover with a manufacturer-approved cover plate that: Matches the shape and size of the junction box, Can be securely fastened (screw holes must align), Is rated for the environment (e.g., damp, outdoor, etc.). 1st Floor - Outdoors SECOND FLOOR AUDIT FINDINGS NO: 2-A CATEGORY: OUTLET AND SWITCH INSTALLATION RISK LEVEL: HIGH FINDING: OUTLET IS MISSING THE FACEPLATE/COVER The live receptacle parts and internal wiring are exposed. IMPROPER MECHANICAL PROTECTION Plug is inserted while the outlet is unsecured, risking movement, loosening, or accidental disconnection. VIOLATIONS: MISSING COVER PLATE PEC Part 1, Sec. 3.10.6.1 (Boxes and Fittings – Covers) - In completed installations, each box shall be provided with a cover, faceplate, or fixture canopy. UNSEALED BOX OPENINGS PEC Part 1, Sec. 3.10.6.2 (Unused Openings) - Unused openings in boxes shall be effectively closed to afford protection substantially equivalent to the wall of the box PEC Part 1, Sec. 1.10.1.11 - Live parts of electrical equipment operating at 50 volts or more shall be guarded against accidental contact. PEC Part 1, Sec. 1.10.1.3 - Electrical equipment shall be installed in a neat and workmanlike manner. OSHS RULE 1210 - Requires electrical systems to be “maintained in a safe condition to prevent danger from accidental contact with live parts.” RECOMMENDATIONS: Install approved outlet faceplate/cover immediately. 2nd Floor - First Lecture Room FINDINGS NO: 2-B CATEGORY: LIGHT FIXTURE RISK LEVEL: HIGH FINDING: UNSAFE LIGHT FIXTURE The fluorescent lighting fixture is surface-mounted to a ceiling section made of discolored/damaged plywood. The plywood appears water-stained and potentially compromised structurally. The light appears to be sagging slightly, suggesting it may not be securely fastened. Ceiling looks patchy and unsafe for electrical equipment installation. VIOLATIONS: NEC 410.36(B) – Suspended or Pendant Luminaires "Luminaires (lighting fixtures) shall be securely mounted so that they will not fall." USE OF DAMAGED, NON-STRUCTURAL MATERIAL NEC 110.3(B) – "Equipment shall be installed and used in accordance with its listing and labeling.” Most lighting fixtures are listed for mounting on solid, non-combustible, structurally sound surfaces. OSHA 29 CFR 1910.303(b)(7) - Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. NEC 110.12(A) – Electrical equipment shall be installed in a neat and workmanlike manner. RECOMMENDATIONS: Immediately secure or remove the fixture until it can be properly mounted. Replace damaged plywood with a code-compliant, structurally sound mounting surface. Inspect electrical connections for signs of strain, wear, or insulation damage due to movement or moisture. Ensure the replacement mounting surface is noncombustible if required (especially for commercial/industrial spaces). 2nd Floor Hallway FINDINGS NO: 2-C CATEGORY: OUTLET AND SWITCH INSTALLATION RISK LEVEL: MEDIUM FINDING: SWITCHES INSTALLED VERY CLOSE TO THE CEILING INSTEAD OF THE STANDARD ACCESSIBLE HEIGHT Installation does not follow standard ergonomic or accessibility guidelines, proves to be inaccessible during emergencies. VIOLATIONS: PEC Part 1, Sec. 1.10.1.2 (Access to Electrical Equipment): “Electrical equipment shall be installed so that sufficient access and working space is provided and maintained about all electrical equipment.” Accessibility Law (BP 344 – Accessibility Law for Disabled Persons) Controls such as switches and receptacles shall be within reach range of persons with disabilities. Standard reach range: 0.9 m to 1.2 m above finished floor (for switches and outlets). RECOMMENDATIONS: Reinstall switches outlets at standard heights: Switches: 1.2 m (approx. 4 ft) above floor finish. Ensure compliance with BP 344 Accessibility Law for reach ranges. Wires appear to be unsupported, loosely draped, and not in proper conduit. One or more conduits are broken or incomplete, leaving conductors exposed. Wires lie across sharp edges and corrugated metal roofing, increasing the risk of insulation damage and electric shock. No visible weatherproof enclosures, junction boxes, or cable supports. Some cables appear to enter the building through unsealed wall penetrations. Improper routing and sagging of wires. VIOLATIONS: EXPOSED OUTDOOR WIRING WITHOUT PROTECTION NEC 300.5(D)(5) – Cables and raceways installed above ground outdoors shall be installed in rigid metal conduit (RMC), intermediate metal conduit (IMC), rigid PVC conduit, or other approved means. NEC 300.9 – Where raceways are installed in wet locations above grade, the interior of these raceways shall be considered a wet location. UNSUPPORTED AND SAGGING CONDUCTORS NEC 300.11(A) – Raceways, cable assemblies, boxes, and fittings shall be securely fastened in place. NEC 334.30 – Nonmetallic-sheathed cable shall be supported and secured every 4.5 ft (1.37 m). RISK OF PHYSICAL DAMAGE NEC 300.4(B) – Cables subject to physical damage shall be protected with conduit or listed fitting. OPEN WALL PENETRATIONS NEC 300.7(A) – Where raceways pass through an exterior wall, they shall be sealed to prevent moisture ingress. NEC 314.25 – Boxes and junctions shall be fully enclosed with approved covers. RECOMMENDATIONS: Replace all exposed wiring with UV- and weatherresistant cables rated for outdoor use. Install all outdoor cables in approved conduit (e.g., PVC, RMC, or IMC) suitable for wet and exposed locations. Secure all wiring using proper supports, clamps, or straps at intervals per NEC (typically every 4.5 ft for NM cable). Replace all broken or incomplete conduit sections to ensure full mechanical and environmental protection. Seal all wall penetrations using approved fittings and weatherproof caulking or boots to prevent water ingress. Install weatherproof junction boxes at all transition points and ensure they are properly covered.
