Case 1
Patient: you have pain in your knee (on the patella ligament just below the apex of patella).
Time: about a week ago. It started gradually. Don’t remember exact onset.
Sports: Basketball. I jump a lot during practices and games.
Any feel: It aches beginning of the practice/w-up. It eases up as I get warmed up. I can also feel clicking feeling
in my knee when I jump before w-up.
Localization:
point at where a red circle is.
Characteristics of Pain: achy and dull during w-up. Scale is 5/10 before w-up, 2-3/10 after w-up.
Influences/associations of pn.: in the morning is the worst time. W-up makes it better.
Previous Hx.: No Hx., no current or recent injuries.
Case 2
Patient: You just rolled your ankle (inversion sprain)
Time: it just happened in the gym while you were practicing. I landed on the other player’s foot after jumping
for a rebound. I rolled inward.
Sports: Basketball
Any feel: It popped when I rolled. I felt immediate pain.
Localization:
Point where a red circle is
Characteristics of pain: Very sharp pain. 7/10 right now.
Influences/associations of pn.: Sharp pain with any movement especially inward (inversion). Tenderness with
touch on the red circle area, swelling around the red circle area.
Previous Hx.: I have rolled ankle but it was the other leg.
Case 3
Patient: You just dislocated your right shoulder.
Time: Just happened during a practice session. I slid from the first to the second base and when I extended my
arm straight out to reach the base, the right side hit it harder, and it popped out of a socket.
Sports: Baseball
Any feel: I felt a pop sound and also felt in my right shoulder. It feels like the shoulder is out of place.
Localization:
It’s painful and I have to hold my forearm with the other hand.
Characteristics of Pain: throbbing pain. The pain scale is 9/10 right now.
Influences/associations of pn.: Constantly throbbing pain in my right shoulder. It causes sharp pain when I try
to move any direction. It’s not tender to touch on the shoulder. But it Feels better when I hold up my right arm
closer to my body with my left hand.
Previous Hx.: No history of shoulder injuries. No current or recent injuries.
Case 4
Patient: You have been experiencing lower back pain since high school (You are now a Sophomore in college).
Time: I had lower back issues and pain since my senior year of high school, and it started bugging me again this
past 2 weeks. It was tolerable then started getting worse a couple of days ago.
Sports: Golf
Any feel: It feels sore and tight around my lower back spine.
Localization:

Characteristics of Pain: achy and dull pain. Sometimes sharp pain shoots down through the back side of my
right leg. The pain scale is 5-6/10 usually, but sometimes it gets 8/10 after practice.
Influences/association of pn.: The pain gets worse when I bend over. Or when I stretch my right leg and feel
sharp pain along with my right leg.
Previous Hx.: I was initially diagnosed with a herniated disc between L4 and L5 in high school.
