participation for up to 12 months.

$6000/youth to assist with various
needs (e.g. rent assistance, food,

Attorney
fills out
referral
form and
submits to
DYD.
Agency reaches
out to attorney
within 48 hours
prior to any
contact with
youth.
DYD staff
identifies
agency and
emails referral
to agency.
Agency reaches
out to youth and
parent or
guardian and will
notify attorney of
any unsuccessful
attempts.
Attorney communicates
with youth/guardian and
obtains and gives verbal
consent to agency to do
outreach.
Agency enrolls
Youth in program
who agree to
participate.
