PARENT/ GUARDIAN CONSENT FORM
I hereby consent to the participation of my child (ren), ___________________________________,
in a visit to an educational trip to Entoto park Museum and Yaya Village on June 19 and 20
2024. I understand that this event will take place away from the school grounds, and my child (ren)
will be under the supervision of a designated school employee. Furthermore, as a parent or legal
guardian, I remain fully responsible for any legal liability that may result from any personal
actions taken by the aforementioned child(ren) and/or any Act of God that may occur.
