3. Responsabilidades - Responsibilities
3.1. Representante do navio
Figura: Profissional qualificado e com conhecimento técnico designado pelo Armador, sob
o qual recai a exclusiva responsabilidade da máquina e/ou equipamento durante todo o
período da consignação.
• Coloca a máquina e/ou equipamento inoperacional de forma a impedir o
funcionamento, operação ou ativação intempestiva durante o período da consignação.
• Verifica a ausência de energia por forma a garantir a eficácia da consignação.
• Sempre que possível aplica o sistema de bloqueio e etiquetagem do navio em
complemento ao da Lisnave.
• Garante a manutenção do sistema de bloqueio e etiquetagem durante todo o período
da consignação.
• Transmite à Gestão de Projetos nas reuniões diárias possíveis incompatibilidades entre
os seus trabalhos e as Consignações em curso.
• Suspende os trabalhos imediatamente, afasta os trabalhadores da zona de perigo e
informa a Gestão de Projetos e a Prevenção e Segurança em caso de violação da
consignação.
3.2. Gestão de Projetos
Figura: Gestor do Projeto ou seu mandatário.
• Envia previamente a listagem de possíveis consignações ao Responsável do Trabalho e
Prevenção e Segurança, assim como a identificação no local.
• Coordena o planeamento dos trabalhos por forma a evitar incompatibilidades e
comunica atempadamente à Prevenção e Segurança.
• Realiza reuniões diárias com o Cliente por forma a coordenar os trabalhos de ambas as
partes, a distribuir e analisar o Registo de Consignações e a questionar o Cliente,
diariamente, sobre eventuais trabalhos da sua responsabilidade que possam interferir
com as Consignações em vigor.
• Autoriza a realização do trabalho e respetiva consignação.
• Colabora na realização da Consignação.
• É o interlocutor com o representante do navio.
• Coordena de modo a que as condições técnicas de execução, mobilização de recursos e
meios bem como a segurança de pessoas e bens estão reunidas.
• Em caso de violação da Consignação, suspende os trabalhos imediatamente, afasta os
trabalhadores da zona de perigo e informa a Prevenção e Segurança.
Nota: Nos casos pontuais, em que trabalhos estejam finalizados e os Responsáveis do
Trabalho / Executantes do Trabalho não se encontrem presentes, a Gestão de Projetos
como entidade responsável pela reparação, pode solicitar a desconsignação. Nestes casos,
a Gestão de Projetos assume a inteira responsabilidade dos Responsáveis do Trabalho /
Executantes do Trabalho, nomeadamente verificar no local a efetiva conclusão dos
trabalhos e assinar o documento de consignação pelos Responsáveis do Trabalho /
Executantes do Trabalho, devendo justificar a razão inadiável para a mesma no campo de
observações da Ficha de Consignação.
3.3. Responsável do Setor
Figura: Encarregado, Chefe de Equipa, Preparador ou Especialista.
• Assegura que as condições técnicas de execução, mobilização de recursos e meios bem
como a segurança de pessoas e bens estão reunidas.
• Solicita a consignação à Prevenção e Segurança.
• Acompanha e verifica o cumprimento dos Processos de Consignação.
• Assegura a manutenção do bloqueio e etiquetagem durante todo o período da
consignação.
• Em caso de violação da Consignação, suspende os trabalhos imediatamente, afasta os
trabalhadores da zona de perigo e informa a Prevenção e Segurança.
• Verifica no local a conclusão dos trabalhos e se estão reunidas as condições para início
do Processo de Desconsignação.
• Informa a Gestão de Projetos da conclusão dos trabalhos.
• Solicita a desconsignação.
3.4. Executante do Trabalho
Figura: Responsável devidamente qualificado designado ou indicado pela entidade
executante para assumir a responsabilidade efetiva dos trabalhos abrangidos por uma
consignação.
• Assegura que as condições técnicas de execução, mobilização de recursos e meios bem
como a segurança de pessoas e bens estão reunidas.
• Acompanha e verifica o cumprimento dos Processos de Consignação.
• Verifica a ausência de tensão antes do início dos trabalhos sempre que estes envolvam
risco elétrico.
• Assegura a manutenção do bloqueio e etiquetagem durante todo o período da
consignação.
• Em caso de violação da Consignação, suspende os trabalhos imediatamente, afasta os
trabalhadores da zona de perigo e informa a Prevenção e Segurança.
• Implementa as condições técnicas para realização da desconsignação.
• Informa o Responsável do Trabalho da conclusão dos trabalhos.
3.5. Prevenção e Segurança
Figura: Técnico de Prevenção e Segurança.
• Analisa o trabalho a executar com o Responsável do Trabalho.
• Acompanha e verifica o cumprimento dos Processos de Consignação.
• Aplica os sistemas de bloqueio e etiquetagem.
• Emite as Fichas de Consignação.
• Dá início ao Processo de Desconsignação após comunicação da conclusão dos trabalhos
pelo Responsável do Trabalho.
• Arquiva as Fichas de Consignação no processo do respetivo navio.
• Envia à Gestão de Projetos, Setores da Produção e Entidade de pesquisa de gases
(quando aplicável) a atualização do Registo de Consignações.
• Suspende os trabalhos se os mesmos estiverem a decorrer de forma insegura ou se
existir violação da consignação (de acordo com a Ordem de Serviço 04/2021).
3.1. Vessel Representative
Person: Competent professional and with technical knowledge, designated by the Owner,
with exclusive responsibility for the machinery/equipment during the entire lock-out/tagout period.
• Place the machinery/equipment inoperative in order to prevent activation or operation
during the entire lock-out/tag-out period.
• Verify the absence of energy in order to guarantee the effectiveness of the lock-out/tagout.
• Whenever possible apply the vessel lock-out/tag-out system in addition to Lisnave’s lockout/tag-out system.
• Ensure the maintenance of the lock-out/tag-out during the entire lock-out/tag-out
period.
• In daily meetings with Project Manager, identified conflicts between their works and the
current Lock-outs/Tag-outs from shipyard.
• Stops the work immediately if the lock-out/tag-out system is violated, move all the
workers away from the danger area or machinery/equipment and inform the Project
Manager and Safety Technician.
3.2. Project Manager
Person: Project Manager or Project Manager Assistant.
• Sends the list of possible lock-out/tag-out to the Person Responsible for Work and Safety
Department, as well as the identification on site.
• Coordinate the work plan in order to prevent any work conflicts and inform Safety
Department.
• Holds daily meetings with the Client in order to coordinate the work of both parties,
identify possible incompatibilities and / or simultaneous operations, distributes and
analyses the Lock-out/Tag-out Record and asks, at a daily basis, the Client for possible
works that can interfere with Lisnave ongoing Lock-outs/Tag-outs.
• Authorize the work and lock-out/tag-out accordingly.
• Cooperate with the execution of lock-out/tag-out.
• Interfaces with Vessel Representative.
• Coordinate in order to ensure the technical conditions, mobilization of resources and
means as well as the safety of people and assets.
• Stops the work immediately if the lock-out/tag-out system is violated, move all the
workers away from the danger area or machinery/equipment and inform the Safety
Technician.
Note: Some cases, when the works are finished and the Person Responsible for Work /
Team Leader / Supervisor are not present, the Project Manager or Deputy as repair
manager, is allowed to request the removal of the lock-out/tag-out. In this case, the Project
Manager or Deputy takes full responsibility to ensure the work completion in place and to
sign the lock-out/tag-out form as Person Responsible for Work / Team Leader / Supervisor.
The Project Manager or Deputy should justify the reason in the remarks area of the lockout/tag-out form.
3.3. Person Responsible for Work
Person: Foreman or Technical Engineer.
• Ensure technical conditions, mobilization of resources and means as well as the safety of
people and assets.
• Request the lock-out/tag-out to the Safety Department.
• Ensure compliance with lock-out/tag-out procedures.
• Ensure maintenance of the lock-out/tag-out during the entire lock-out/tag-out period.
• Stops the work immediately if the lock-out/tag-out system is violated, move all the
workers away from the danger area or machinery/equipment and inform the Safety
Technician.
• Confirm in place that the work is completed and check that is safe to remove the lockout/tag-out.
• Inform the Project Manager that the works are completed.
• Request the lock-out/tag-out removal.
3.4. Team Leader / Supervisor
Person: Competent professional and with technical knowledge, designated by the Working
Company that will be responsible for the works cover by the lock-out/tag-out.
• Ensure technical conditions, mobilization of resources and means as well as the safety of
people and assets.
• Ensure compliance with lock-out/tag-out procedures.
• Ensure maintenance of the lock-out/tag-out during the entire lock-out/tag-out period.
• When working on electrical equipment/systems, verify the absence voltage before and
during the work.
• Stops the work immediately if the lock-out/tag-out system is violated, move all the
workers away from the danger area or machinery/equipment and inform the Safety
Technician.
• Apply the technical conditions for the lock-out/tag-out removal.
• Inform the Person Responsible for Work that the work is completed.
3.5. Safety Technician
Person: Yard Safety Technician.
• Analyse the work to be performed with the Person Responsible for Work.
• Verify and ensure compliance with lock-out/tag-out procedures.
• Apply the lock-out/tag-out.
• Issue the lock-out/tag-out forms.
• Removes the lock-out/tag-out when the work is completed and requested by the Person
Responsible for Work.
• Archives the Lock-out/Tag-out Forms at the vessel file.
• Send the Lock-out/Tag-out Record updates to the Project Manager, Production and
Competent Chemist Entity (when applicable).
• Stops the work immediately if lock-out/tag-out system is violated or if the work is being
performed in a unsafe/dangerous way (according to Ordem de Serviço 04/2021).
