This is to inform you that you are one of the respondents of the School-Based Action Research
“IMPROVING MATHEMATICS ACHIEVEMENT AND POSITIVE ATTITUDES OF STRUGGLING STUDENTS
THROUGH GAME-BASED AND ACTIVITY-ORIENTED ASSESSMENT”. Kindly answer the following set of survey
questionnaire that measures your level of appreciation regarding your Mathematics subject. Rest assured that

Positivity in Mathematics
Mathematics is a very worthwhile and necessary subject.
Mathematics plays an important role in my life’s work and decision making.
I study Mathematics because I know how useful it is.
Mathematics contributes a lot to make my life meaningful.
When I get difficulty in understanding Mathematics, I am still optimistic that
my skills in solving problem will improve.

Nearly everyone is capable of learning Mathematics if he works at it.
Relation to Real World
I study Mathematics to learn things that will be useful in my life outside of the
school.
Mathematical concepts express meaningful relationships among countable
things or amounts.
Learning Mathematics changes my ideas about how the world works.
Mathematics enhances my reasoning skills which are necessary for my daily
life activities.
Mathematics has something do with what I experience in the real world.
To understand Mathematics, I sometimes relate my personal experiences to
the topics being studied.
Importance of Formulas
I’m satisfied when I am solving problems by the use of a certain formula.
It is important for me to know where Mathematics formulas come from.
I am interested to understand how formulas are applied in problem solving.
I can easily solve Math problems through mastering formulas.
It is interesting in my part to derive another formula from the original one.
It’s easy for me to memorize Mathematics formulas.
I enjoy compiling Math formulas and equations in my notebook.
Dependence on Procedures
I believe that there are many procedures in solving a Mathematical problem.
If my computation in Math problem gives a result which is very different from
what I’d expect, I go over the procedures or steps to check the answer.
It is a great help for me to procedures of the solutions to sample problems in
preparation for a Math test.
I have to memorize and understand the given procedures to prepare for a
Math test.
I hardly solve Math problems without looking the given examples and
procedures.
When my teacher introduces a Math problem, I am excited for the procedures
on how to solve it.
Confidence in Solving
I am confident that I can solve the problem after studying the related
examples.
I don’t feel nervous talking out-loud about Math principles in front of my
classmates.
I am eager to participate in discussion that involves Mathematics.
I am comfortable to show my solution in Math class.
If I get a low score in Math quiz, I am confident that I can cope up in the next
test.
I like to show my solution on the board during class participation or share my
Math skills with peers in the class.
Exploration in Problem Solving
I enjoy solving Math problems.
I love scanning Math books to look for Math problems related to our lessons.
When I see Math problem, I am excited to solve it.
I appreciate hearing the thoughts and ideas of my classmates in Math class.
I cannot concentrate if I cannot solve Math problem assigned to me.
I utilize Math formulas, procedures and online applications to explore my
abilities in solving problems.

I prefer working alone rather than in groups when doing Math problem.
