People with disabilities would want to lead as normal a life as possible and to
contribute to society as much as they can.
 Objective

Rehabilitation services aim to acknowledge the equal rights of people with disabilities
to be full members of the community by assessing them in developing their physical,
mental and social capabilities to the fullest possible extent and by promoting their
integration into the community

 Go hand in hand with “ Convention on the Rights of Persons
with Disabilities”. 聯合國的殘疾人權利公約 Opinions and
wills of PWD should consider when making decisions
 When assessing needs of service users and formulating year
plans, we consider their likes, strengths and wishes, and
prioritize them into the year plan whenever feasible.
 Eliminates the bio, psycho, social barriers that service users
face in society
 Acquire relevant information through empathy and
observation of caregivers (frontline staff or family members).
 Service users participate in making decisions about their
intervention plan, they have higher motivation and achieve
better results.

 An evaluation framework that takes care of the
comprehensive needs of service users through cross
disciplinary communication , while providing tailor-made
service
 It takes a “holistic” approach to understand disability
 Evaluate the capabilities and needs of each service user
based on the functional status of six factors
 PWDs have limited cognitive / physical abilities, with
appropriate assistance or support, they can demonstrate
their unique capabilities and even contribute to society

 Cross disciplinary collaboration 跨專業協作
 Regular communication among professionals such as
social workers, nurses, occupational therapists,
physiotherapists, clinical psychologists, speech
therapists…Common Language 共同語言 貫通各職系，提升對殘
疾人士應有的理降
 Work on common documents 共同文件應用ICF框架所製訂的共
同文件，從各專業及不同職系職員收集服務使用者的喜好、意願、及身體方面 (身體狀
況、身體肌能及結構)、社會方面 (活動、參與、環境因素) 和心理方面 (個人因素) 的資
訊，作全人評估，為服務使用者度身訂做適切的年度計劃 (包括介入目標、介入方案及
了解介入成效)

services for PWD.
 At the end of March 2022, there were 17,502 pre-school places, 19,559 day
places and 15,204 residential places.
