City of Sta Rosa ) AFFIDAVIT OF NON-INTEREST TO FILE CHARGES AGAINST THE SUSPECTS I, Christian Montano Perfania, 25 years old (DOB: May 20, 1999), single, checker, native and resident of Sarmiento Compound, Purok 5, Brgy Pulong Sta Cruz, Sta Rosa City, Laguna after having been sworn in accordance with the existing laws, do hereby deposes and say that: That, I am the victim of frustrated murder (shooting incident) case that transpired at about 06:25 PM of October 24, 2017 at Sarmiento Compound, Purok 5, Brgy Pulong Sta Cruz, Sta Rosa City, Laguna; That, the suspect in the said case was identified as Mateo Cailo y Flor, 43 years old (DOB: March 19, 1980), single, jobless, native of Catbalogan, Samar and resident of Bagong Sta Rosa Village, Brgy Pooc, Sta Rosa City, Laguna; That, the incident was reported to and investigated by Santa Rosa City Police Station, Brgy Tagapo, Sta Rosa City, Laguna; That, I am hereby signified that I am no longer interested to file charges against the suspect and as a consequence thereof, I hereby willingly, voluntarily and without use of violence, threat, force and intimidation withdraw the said complaint and waived the whatever rights I have under the law and equity against him; That, I am knowledgeable of the bestowed rights under the law and that I am completely understood the consequence of this affidavits; That, I am executing this affidavit, willfully and voluntarily without the means of violence, threat, force and intimidation used against me to do the same; That, I executed this affidavit to attest the truthfulness of the above facts and for all legal intent and purposes it may serve.
