Uyag, RN Nursing Care Plan ASSESSMENT Subjective: “Medyo masakit pa po tapos lalong namumula kapag natatamaan ng liwanag. Objective: - Redness of the eye - Eye is watery. - Unequal size of the eye due to inflammation. DIAGNOSIS Risk for infection related to s/p pterygium eye surgery. PLANNING After 8hrs of nursing intervention the patient will be able to: - Verbalize the lessen of redness and inflammation of the eye. - Know the proper intervention to avoid infection. After 1-2 weeks of nursing intervention the patient will be able to: Apply the intervention properly even at home. - Display healing when conducting follow-up checkup. INTERVENTION Position the patient in a comfortable position and not lift heavy objects. Advise the patient to wear sunglasses or eyepatch especially on a daylight. Advise the patient to not put any eye make-up. Always wash hands before touching the eye. Avoid going to places where dust and dirt is present. Advise the patient to not scratch the eye even it feels itchy. Apply eye drops and administer medications such as antibiotics as indicated by the physician. RATIONALE To avoid putting stress and strain on the surgery site. To reduce the exposure of the eye to sunlight. To reduce the risk for infection. To prevent transfer of microbes. To reduce the exposure to dirt to avoid infection. To avoid irritation of the eye. To help surgical site heal faster. EVALUATION After 8hrs of nursing intervention the patient was able to: - Verbalize the lessen of redness and inflammation of the eye. - Know the proper intervention to avoid infection. After 1-2 weeks of nursing intervention the patient was able to: Apply the intervention properly even at home. - Display healing when conducting follow-up checkup. Goal was met.
