2023 BLS Medication update 2023 SWTP BLS update • April 1st 2023 the commonwealth has added some medications to the protocols for the BLS provider. • Diphenhydramine (Benadryl) • Acetaminophen (Tylenol) • Ibuprofen (Advil, Motrin) 2023 SWTP update • Along with the addition to the medications the state will be requiring that we carry additional Naloxone HCI (Narcan). AAS will be supplying this in the single dose (4mg) spray. You must find out about any allergies prior to administrating any medication! SWTP 2.2A Allergic reaction / anaphylaxis adult SWTP 2.2A • EMT Standing orders Mild distress: Diphenhydramine 25-50mg by liquid or oral tablet / capsules Severe distress: Epi auto injector 0.3mg or IM in accordance with protocol 6.5 check and inject Epinephrine by EMT – basics. SWTP 2.2 p (pedi Allergic / Anaphylaxis) • When administering epinephrine IM, EMTs may do so in accordance with protocol 6.5 check and inject. • Diphenhydramine – 1mg/kg for patients 2 years or older, up to a total dose of 25mg. (preferably by liquid measured dose) Diphenhydramine Concentration 12.5mg per 5mls dosing cup provided Diphenhydramine dosing Adult: 25-50mg by oral liquid (dosing cup provided with container) Pediatric: 1 mg/kg for patients 2 years or older (measured dosing cup provided). IE 12kg pt would receive a 12mg or 5MLs. SWTP 2.13 Pain & Nausea management SWTP 2.13 • EMT STANDING ORDERS: • 1.O ROUTINE PT CARE • Ibuprofen: 600mg by liquid or oral tablets / capsules (AAS will be using liquid with provided measuring cup) • Acetaminophen: 650 – 1000 mg by liquid or oral tablets / capsules (AAS will be using liquid with provided measuring cup) Acetaminophen Concentration: 160mg per 5ml Ibuprofen Concentration: 100mg per 5ml Naloxone (Narcan) SWTP change: Increased limit of dosing of Naloxone for all levels to 8mg. Armstrong will now be supplying each ambulance with 20mg of Naloxone in accordance with state regulations. You will be seeing a new dosing and delivery device. Naloxone (Narcan) 4mg single dose metered nasal spray Naloxone cont. • Note: Per SWTP 2.14 Naloxone should only be administrated in suspected overdose patients with inadequate respiratory rate. Treatment should progress toward the restoration of adequate respirations. Patients with inadequate respiratory rates may need to be ventilated until their respiratory rate increases. Naloxone cont. Hold the spray with your thumb on the bottom of the plunger and your first and middle finger on either side of the nozzle. Naloxone cont. Guidelines for use of single metered dose nasal spray. • 3. Gently insert the tip of the nozzle into either nostril and firmly press the plunger to give the 4mg dose of Naloxone. BLS medication update. Soon you will be seeing a new medication kit on all BLS units. These will be clear boxes that are sealed with expiration dates visible. Should you use any item from the box please contact a FOM and do a supply request for replacements. The single dose metered naloxone spray will be kept in the first in bag.
