Students are required to submit this form to respective Supervisor within 4 weeks before Final Examination.
C.
Course Registration will be registered as In-Progress (IP) by Faculty Executive during Week 1 of the new semester and
confirm on behalf of the student.
D. Student who failed to submit this form within the stipulated time frame will be given an F (Failed) Grade for the current
semester. Full charges will be imposed for a REPEAT course.
