Description of a report

Equipment/system name:
HT-P-4101A: Lube oil pump of HT-K-4100

Status of equipment/system before inspect: (attach evidence, P&ID, etc. if needed for clarity)
Date 26/02/2024, while collect data for all rotating equipment on PQP that running hours over 40.000
hrs, EnI found Motor HT-PM-4101A has abnormal vibration signal.
And then taking ultrasound data for the pump HT-P-4101A has a louder noise than the pump HTP4101B, it is necessary to inspect the condition of Lube oil Pump HT-P-4101A.
Total running hours of HT-P-4101A: 44000 hrs

Visual check bearing found the bearing locking device has loosen and the inner bearing
has light scratches, may cause Bearing and rotor shaft start to down grade.
Step 6
After EnI install the new motor, run test the pump in force mode and collect
parameter data.

For MR Lub. Oil pump A (HT-P-4101A): after EnI replaced the motor, the pump run
test and investigated with Mr. LongVH, evaluate the pump head for signs of damage,
suspecting the DE side bearings. EnI team has replaced the spare motor, signs of
ultrasound impact still appear on the pump.

Install new pump , old pump transfer to Mech WS
Old pump was dismantle in Mech WS, found defect on pump shaft at DE bearing
position
Evaluate: the old pump head of HT-P-4101A was defected shaft at DE bearing position
4. Status of equipment/system after inspected, finding: (attach evidence, P&ID, etc. if needed for
clarity)
Install new pump for using.
Old pump was defected shaft at DE bearing position, will send onshore to repair for back up
5. Root cause: (if found)
Direct root cause: Old pump was defected shaft at DE bearing position.
6. Recommendation:
- Send old pump to onshore to repair by chopper 17/4/2024.

Disposal located: N/A
Useable spare part: Old pump will send to shore for refurbish and then replace for oil pump B (HT-P41010B). After that refurbish the old pump and spare for HT-P-4101A/B
