2016 Tool Box Talk- 1st Quarter, Week 1: 12/28/15
This Tool Box Talk provides information on safety documentation.
Safety, Quality, and Production is a three legged stool.
Without one the other two fail. For every project to get
started out on the right foot, it begins with Safety.
Safety Documentation Responsibilities
Project Manager:
Prior to the start of the project, the PM is responsible for the
creation of the job file or “blue book.” Within the “blue book”
the PM is responsible for:
1. The Project Safety Plan
This is the PM’s initial safety assessment of the project
based on the technique to be performed, general contractor
requirements, and any site visits performed prior to the
beginning of work. After the PSP has been completed it will
be reviewed and signed off on by at least two of the
following; Safety Manager, Operations Manager,
Branch/Project Manager, or the Superintendent.
The PSP must be site specific and include unique hazards
present at the sight. For example, if micropiles are being
installed right next to a river, then the PSP should include
a section on working near water and the hazards
associated with it. Your regional safety coordinator can
assist you if any questions arise
3. Job Hazard Analysis
Also required in the Job File or “Blue Book” is the Job
Hazard Analysis or JHA. JHA’s have been developed for
each technique that is performed. The PM shall also
adjust the JHA for the project, to ensure that it is
project/site specific.
2. Review the Job Hazard Analysis (JHA) with the entire
crew. Have the crew sign the final page of the JHA.
4. Identification of emergency medical facilities
The PM is responsible for locating the closest approved
medical facilities for the project. Use Travelers online to
find these facilities.
3. Review the approved medical facilities with the crew
and post the list in a conspicuous location.
4. Review the location of any utilities on-site and the
method of marking and protecting the utility.
5. Once the information for the project has been
reviewed with the crew they shall all sign the Project
Crew Safety Agreement.
5. Utility Locate
The PM shall apply for the utility locate ticket. Once the
response is received the ticket shall be included in the
“blue book”. If there are utilities on-site the location and
the type of utility shall be reviewed with the project
superintendent.
Superintendent Responsibilities
Once the Project Manager has completed the job file it
will be reviewed with the Project Superintendent at a
pre-job meeting. The Project Superintendent has the
following responsibilities that must be completed prior to
beginning work on the job.
1. Review the PSP with the crew (including the PSP
initial Assessment.
