TECH489 Participation Report Form There are three parts to this report. The first part is the team activity and the second part is the individual activity. The third part is the individual team member rating based on leadership and teamwork. The person designated to fill out the report must fill in the team activity and gather the individual activities to include in this report. As a team decide the best way to rate the leadership and teamwork activities. Keep in mind, your instructor will also be observing the team interaction so that the final grade decision is made by the instructor. Part 1 – Team Activity 1. In sensitive situations, the instructor may meet with team members collectively or individually to ask for more information. In all cases, the grading decision of the instructor will be final. In each phase of the project, students whose evaluation score is: ● 5 will earn full participation points and 100% of all the team assignment grades in this phase. ● 4 will earn 45 participation points and 85% of all the team assignment grades in this phase. ● 3 will earn 35 participation points and 75% of the team assignment grades in this phase. ● 2 will earn 25 participation points and 65% of all the team assignment grades in this phase. They will also need to meet with their instructor and commit to strategies for improvement. ● <2 will earn 0 participation points and 0 points in the team assignment grades. They will also need to meet with their instructor to commit to improvement strategies in order to avoid being dropped from their team.
