I hereby acknowledge that I received the above equipment in good condition. It is my responsibility to
surrender the said equipment within 15 days upon either my separation from the company or being
issued a Return-to-Work Notice. Failure to do so gives the company the right to automatically deduct
the cost of the equipment from either my next scheduled payout or my final paycheck, or require me to
pay in cash if the said equipment is of higher value.
Note:
An employee proven of tampering the equipment and removing the serial number label will be
subject to progressive disciplinary sanction, up to and including termination.
