(PREGNANCY/IMPREGNATES POLICY)
This Agreement is entered into on this day ___________________________by and between
Parent/Guardian/Student

___________________________________________hereinafter referred to as the "Parent/Guardian 1"; And
Parent/Guardian

__________________________________________,
residing
at
___________________________________________hereinafter referred to as the "Parent/Guardian 2";
collectively referred to as the "Parents." And, SAINT FRANCIS COLLEGE, GUIHULNGAN, NEGROS ORIENTAL,
INCORPORATED, an educational institution located at Bateria, Poblacion, Guihulngan City, Negros Oriental,
Philippines hereinafter referred to as the "School."
WHEREAS, the Parents/Student has enrolled their daughter/son named, ________________
______________________________________ hereinafter referred to as the "Student," in SFCG-NOI.
WHEREAS, the Parents/Student and the School acknowledge the importance of providing a conducive and
focused educational environment for all students.
NOW, THEREFORE, in consideration of the mutual covenants and agreements contained herein, the
Parents/Student and the School hereby agree as follows:
The Parents/Student acknowledges that the School has a policy that restricts the enrollment or continued
attendance of students who are pregnant/impregnated during their time of study at the School.
The Parents/Student confirms that they understand and accept the School's policy regarding pregnancy.
The Parents/Student agrees not to enroll or allow the Student to attend the School if she becomes pregnant
during her enrollment or he impregnates during his enrollment.
The Parents/Student understands that in the event the Student becomes pregnant/impregnated while enrolled
in the School, the School reserves the right to terminate the Student's enrollment.
The Parents/Student acknowledges that the School's pregnancy policy is implemented to ensure the well-being
and safety of all students, maintain an uninterrupted learning environment, and comply with applicable laws
and regulations.
The Parents/Student agrees to inform the School immediately if the Student becomes pregnant/impregnated at
any point during her/his enrollment.
The Parents/Student acknowledges that any violation of this Agreement may result in the termination of the
Student's enrollment at the School.
This Agreement represents the entire agreement between the Parents/Student and the School regarding the
pregnancy policy, and supersedes any prior discussions, understandings, or agreements, whether written or
oral.
This Agreement shall be directed by and understood in accordance with the rules of the School.
IN WITNESS WHEREOF, the Parents/Guardian and the School, intending to be legally bound, have executed
this Agreement as of the date first written above.
