Patient Complaints:
Patient is 27 year-old female who presents with bilateral hand pain and numbness.
NCV & EMG Findings:
Evaluation of the Left median motor and the Right median motor nerves showed normal distal onset latency
(L2.7, R2.8 ms), normal amplitude (L7.6, R10.2 mV), and normal conduction velocity (Elbow-Wrist, L66, R60
m/s). The Left ulnar motor and the Right ulnar motor nerves showed normal distal onset latency (L2.3,
R2.12ms), normal amplitude (L9.8, R8.0 mV), normal conduction velocity (B Elbow-Wrist, L72, R64 m/s), and
normal conduction velocity (A Elbow-B Elbow, L64, R69 m/s). The Left median sensory and the Right median
sensory nerves showed normal distal peak latency (L2.6, R2.8 ms), normal amplitude (L42.8, R57.1 µV), and
normal conduction velocity (Wrist-2nd Digit, L54, R50 m/s). The Left ulnar sensory and the Right ulnar
sensory nerves showed normal distal peak latency (L2.3, R2.6 ms), normal amplitude (L56.4, R51.9 µV), and
normal conduction velocity (Wrist-5th Digit, L61, R54 m/s). The Left median/ulnar (dig IV) comparison nerve
showed prolonged distal peak latency (Median Wr, 3.4 ms), normal distal peak latency (Ulnar Wr, 2.9 ms), and
abnormal peak latency difference (Median Wr-Ulnar Wr, 0.5 ms). The Right median/ulnar (dig IV) comparison
nerve showed normal distal peak latency (Median Wr, 2.8 ms), normal distal peak latency (Ulnar Wr, 2.8 ms),
and normal peak latency difference (Median Wr-Ulnar Wr, 0.0 ms). The Left median/ulnar (palm) comparison
nerve showed normal distal peak latency (1.8 ms).
Impression:
The above electro diagnostic study reveals evidence of a bilateral carpal tunnel syndrome (median nerve
entrapment at wrist) affecting motor and sensory components.
1) Right carpal tunnel syndrome of Moderate intensity.
2) Left carpal tunnel syndrome of Mild intensity
Recommendations:
Correlation with clinical findings is suggested.
