You are hereby designated as Health Clinic Coordinator of Ginagdanan Elementary School
effective immediately. As such, you shall perform the following functions/tasks:

Organized a functional Health Clinic;
Orient the officers and members of the club on the objectives, activities,
and projects of the club;
3.
Provide Guidance to the officer and members of the club for the attainment of the
objectives of the club;
4.
Meet the officers at least once a week for planning and evaluation;
5.
Oversee/supervise the activities of the club;
6.
Act as consultant/adviser in all the activities undertaken by the club;
7.
Encourage the participation of the club in school programs;
8.
Submit report related to Health Clinic activities;
9.
Lead the effective implementation of Health Clinic programs and activities;
10.
Design program and schedule of activities for the yearly conduct of School-based
Health Clinic Camp.
It should be understood, however, that your designation is not considered a part of your
teaching load.
This designation takes effect immediately and will remain in force unless revoked by the
proper authority.
