Increased 401(k) Company Match: CGI is increasing the 401(k) matching contribution to 50% of up to 6% of a
members eligible compensation per pay period up to an annual maximum of $6,000.
Dental Benefits: Increasing the annual maximum benefit for the Essential option from $1,000 to $1,500 and from
$1,500 to $2,500 for Select option. The Max Over benefit is eliminated for both options.
Vision Benefits: Enhancing UHC vision by reducing copays from $20 to $10 and increasing framing allowance
from $130 to $150 for in-network benefit. Discontinuing VSP vision option.
Increased Parental Leave: CGI is doubling paid parental leave from 2 week to 4 weeks at 100% of pay for all nonbirth parents that are members of CGI.
Health and Welfare Benefits
Medical Insurance
CGI offers two comprehensive medical plan options administered by Cigna. Both options feature a deductible that
must be met before the plan begins to pay benefits and both pay higher benefits for in-network services. Qualified,
in-network preventive care (e.g., annual physicals, immunizations and age-appropriate screenings) is covered at
100% with no copayment or deductible requirement. To determine if a provider is in one of Cigna’s networks go to

Who’s Eligible?
Regular full-time and regular part-time CGI members scheduled to work 20 or more hours per week are eligible to
enroll in our U.S. benefit plans. You also may enroll your eligible dependents, including your spouse or domestic
partner, a dependent child up to age 26 and a handicapped adult-dependent child. Proof of eligibility may be
required. You must ensure that any individual you enroll in a CGI plan remains eligible under the plan at all times.
When you are eligible for CGI benefits, you may add a qualified same- or opposite-sex domestic partner and a
domestic partner’s child or children to your CGI medical, dental or vision coverage. You must meet the
requirements and return a notarized copy of the CGI Domestic Partner Affidavit, along with the requested
supporting documentation and tax certification, every year. Request the form by opening a case on the HR Service
Center and assigning it to the Benefits – U.S. provider group. You have 31 days after the enrollment deadline to
provide the affidavit and documentation. Domestic Partner coverage cannot be activated without notarized and
verifiable documentation.

2023 Medical Rates and Monthly Surcharges
Important surcharges note: If you are a tobacco user, you will be charged an additional $50 a month for CGI
medical coverage. You must not have used tobacco products in the last six months to qualify for the non-tobacco
user rate. If your spouse/ domestic partner is employed and can obtain medical plan coverage through his or her
employer, you will be charged an additional $100 a month to cover him or her under CGI’s medical option.

How the Consumer-Driven Health Plan (CDHP) Health Savings
Account Works
If you enroll in either CGI medical options, you will have the opportunity to fund a personal, tax-free health savings
account (HSA) for future healthcare expenses. Contributions to your HSA are tax-free and you can use the account
to pay for out-of-pocket medical expenses on a tax-free basis. All money in the HSA is yours and is fully vested as
soon as it is deposited, you can also allow your balance to grow tax-free year after year. Once you turn 65 years
old, you can use your HSA funds as additional retirement income. If you are a continuing participant and already
have established an account at Optum Financial, you do not need to open another. If you would like to open an
HSA account with Optum Financial, please contact Optum Financial at 1-866-234-8913 and use CGI’s group
#3311716.

Well-being Incentives
As partners in your health, CGI offers you an opportunity to earn additional company contributions to your HSA for
completing specific well-being activities. Well-being promotions are at various times during the year. Your ability to
earn all four incentives depends on (1) being enrolled in a Cigna medical option sponsored by CGI; and (2) being
employed during the period that a particular incentive program event is earned and paid. Members who are not
enrolled in a CGI-Cigna medical option can still earn one of the incentives and receive a taxable payment for it
through payroll; provided they are employed during the time the incentive is earned and payable.
Note: The total amount of HSA contributions from all sources cannot exceed IRS annual limits (IRS Code Section 223). CGI’s
well-being contributions count toward the IRS annual limit. Also, take into consideration any HSA funding made by your spouse
or your spouse’s employer. You will be subject to taxes and penalties if you contribute more than the IRS allowable limit. It is
your responsibility not to exceed the annual allowable limit.

Dental Insurance
CGI offers two dental options administered by Delta Dental of Virginia. Delta Dental offers the nation’s largest
dental network, covering nearly 80% of dentists nationwide. To determine if a provider is in one of Delta’s networks

Retirement and Financial Security Benefits
401(k)
The CGI 401(k) Savings Plan offers you the ability to save for retirement and reduce your federal and most state
and local taxes. You can contribute pre-tax dollars and receive a matching contribution from CGI each pay period
that you participate in the plan. CGI will match your contribution at 50% of the first 6% of your salary that you
contribute each pay period, up to an annual maximum of $6,000.
Flexible Spending Accounts
Flexible Spending Accounts (FSAs) allow you to use pre-tax dollars that you set aside from each paycheck to pay
for certain eligible expenses, saving you money in taxes. CGI offers four types of FSAs: Health Care FSA (for
members not contributing to a Health Savings Account), Dependent Care, Limited Purpose (Dental and Vision Only
FSA for members contributing to a Health Savings Account) and Commuter.
Life/Accidental Death and Dismemberment Insurance
CGI provides members basic life and accidental death and dismemberment insurance equal to 1 times their annual
base salary from a minimum of $20,000 to a maximum of $750,000. CGI also provides members the opportunity to
purchase (voluntary) supplemental life and accidental death and dismemberment coverage at competitive group
rates. Members may elect coverage for themselves, their spouse/domestic partner and/or eligible dependents.
Members pay 100% of the cost for supplemental coverage.
Short-Term Disability
CGI provides a short-term disability benefit that provides income protection in the event that a member is unable to
work due to disability. After a two-week waiting period, the plan pays 100% of a member’s gross base pay for four
weeks of disability and then 70% of a member’s gross base pay for weeks seven through thirteen.
Long-Term Disability
CGI provides a long-term disability benefit that provides a member with income in the event that an accident or
illness extends beyond the period covered by short-term disability. The benefit amount paid is based on 40% of the
member’s gross monthly base pay. A Buy-Up option is available that provides an additional 20% of base pay, for a
total of 60% of base pay, up to the plans maximum benefit.
Identity Theft Insurance
CGI offers access to identify theft protection through Allstate, a comprehensive program that protects against one
of the fastest-growing crimes today—identity theft.
SmartDollar
CGI offers members a program to help manage personal finances and plan for their financial future. SmartDollar is
a free and confidential financial planning tool that will help members take control of their money, get out of debt,
save and invest with a proven step-by-step plan.

PNC Workplace Banking
PNC offers a unique bank-at-work program that can help make your financial life easier. Members can earn certain
offers and rewards on select banking products and services when you open and use a qualifying checking product.
You also gain access to the insight and experience of PNC financial specialists.
Employee Ownership Benefits
Share Purchase Plan (SPP)
CGI’s Share Purchase Plan (SPP) puts into practice the company’s philosophy of intrapreneurship through
ownership. This is why all regular members of CGI, both full-time and part-time, are invited to participate upon the
date of hire. Through the SPP, you can become an owner by purchasing shares of CGI stock and receiving CGI’s
dollar-for-dollar match on your purchase, up to 3.5% of your salary for managers, and up to 3.0% of your salary for
non-management level members.
Additionally, members can purchase CGI shares through payroll deduction, up to a maximum of 10% of your
salary. These share purchases are not matched by the company but can be bought and sold without transaction
fees.
Lifestyle Benefits
Adoption Assistance
CGI offers adoption assistance that provides members with financial reimbursement of up to $3,000 per adoption
for qualified expenses (e.g., agency and placement fees, travel expenses associated with adoption, court costs and
legal fees). For more information contact your human resources representative.
Auto & Home Insurance
As a CGI member, you have access to auto and home insurance through Farmers Insurance. This program
provides you with special savings, outstanding customer service and a full suite of products to meet your diverse
insurance needs. In addition to auto and homeowner’s insurance, Farmers offers a variety of discounts on other
policies including: condo/renters, personal excess liability, boat, motorcycle, RV and personal property.
Bright Horizons: Back-up Care and Enhanced Family Supports
Back-up Care: CGI provides members and their families access to Backup Care Options, a program that provides
temporary or short- term care for loved ones of any age when regular caregiving arrangements fall through. Sixty
hours of back-up care are available per year.
Enhanced Family Supports: Bright Horizons Enhanced Family Supports offers free premium access to Sittercity,
a fantastic solution for finding babysitters, virtual sitting, pet care providers and housekeepers. The program also
includes discounts on a local, high-touch nanny placement service for full-time child care. Plus, access elder

caregiving resources, learning pods, discounts on academic support, tutoring, and test prep services and special
privileges for full-time child care, such as preferred enrollment at Bright Horizons.
Member Assistance Program (MAP)
The MAP provides all CGI members, as well as their families (spouse, children and parents living at the same
home address), counseling and support for their personal and professional challenges. It is free and entirely
confidential.
Oxygen Well-being Program
The U.S. Oxygen Program provides a support structure that facilitates health and well-being initiatives and fosters
a culture of wellbeing by giving members opportunities to make healthy lifestyle choices to improve their health and
well-being.
Pet Insurance
CGI offers pet insurance through two providers. Pet Benefit Solutions offers members multiple benefit plans, such
as prescription savings, veterinary discount and comprehensive insurance. Nationwide Pet Insurance provides
protection for your pet when he or she needs it most.
Rethink Benefits
CGI makes available, Rethink Benefits, to members caring for a child with learning, social, or behavioral challenges
or developmental disability. This free benefit gives families 24/7 access to tools and resources to help understand,
teach, and communicate better to your child with special needs.
Tuition Assistance
CGI’s Tuition Assistance Program reimburses for approved degree programs at accredited colleges and
universities, up to $5,250 per fiscal year. Members must apply during the annual tuition assistance enrollment
period to be approved for the subsequent fiscal year and must reach their one-year anniversary before being
reimbursed for classes (manager approval required).
529 College Savings Plan
CGI offers a 529 College Savings Plan to allow members to take advantage of state-sponsored plans that are
designed to help you save for the payment of college tuition for yourself, your children, your grandchildren or other
close relatives.

CGI provides sick leave and vacation pay. Vacation is accrued on a per-pay-period basis and the accrual level is
determined by the member’s years of employment.
Other Paid Leave
Paid leave is provided separately for instances of jury duty, subpoena duty, bereavement, parental and reserve
military training.
This brochure highlights some of the benefits provided at CGI to regular full-time and regular part-time members
who work at least 20 hours per week. Benefits generally begin on a member’s first day of work. Complete details
and provisions of all CGI benefit programs are outlined in the full version of the U.S. Benefits Guide and the official
summary plan descriptions for each plan. Note that official benefit documents take precedence over any
