2023 DISTRICT ATHELETIC MEET

1.
In line with the preparation on the forthcoming EDDIS IV Athletic Meet on different sports disciplines,
this is to inform all concerned that District Athletic Meet for School Year 2023-2024 shall be held on
December 6 and 7, 2023 in the different chosen venues stated in Enclosure No.1.
2.
Prior to District Athletic Meet, it is expected that all units shall conduct school meet to select the best
athletes/teams for higher level of competitions.
3.
This activity primarily aims to select best athlete/s that will represent the district for the higher level of
sports competition and integrate the value of sportsmanship.
4.
The athlete’s eligibility are the following;
a. Birth Certificate- Xerox Copy only (PSA if available) (Cut-off age- January 01, 2007 up)
b.Parent’s Permit
c. Form-137 - Xerox Copy only but certified thru copy by the school head.
d.Medical Certificate
e.Form AR-1
f. Gallery
g. Certificate of Enrolment
h. Sworn Statement, Medical & Certificate of Employment (For the Coach & Chaperon only )
5.
Participants on the said events shall bring their available sports’ equipment’s for the said activity.
6.
Enclosure No. 2 contains the list of officiating officials and enclosure no. 3 for different working
committees or event assignments.
7.
Punctuality in attendance should be observed strictly by the participants in order to avoid unnecessary
delays. Opening program shall start 7:30 a.m.
8.
Expenses to be incurred such as meal and snacks allowance for the members of work committees,
contestants and coaches, materials and other incidental expense shall be taken from Local School Fund
subject to the usual accounting and auditing rules and regulations.
9.
Immediate and wide dissemination of this memorandum is earnestly desired.
