The Philippine Health Insurance Corporation or widely known as
PhilHealth is a Government Owned and Controlled Corporation
(GOCC) created through the National Health Insurance (NHI) Act of
1995 or Republic Act 7875. It is the administrator of the National
Health Insurance Program (NHIP) which was established to provide
health insurance coverage for all Filipinos and ensure affordable,
acceptable, available and accessible health care services for all
citizens of the Philippines.

1. ONLINE/ E-MAIL SERVICES
Visit www.philhealth.gov.ph, download & print PMRF.
Completely fill out the PMRF.
Send the scan copy of the filled-out PMRF together with
other required documents via email account of PhilHealth
office in your area. (sflhiomembership.online@gmail.com)
2. P-LINK
Fill out PMRF to be given by the hospital clerk or facility
where you are admitted.
Submit it back to the clerk with a copy of valid ID, birth
certificate, & other supporting documents to the hospital
clerk for processing.
3. DROPBOX SYSTEM/ FRONTLINE SERVICES
Submit the completely filled out PMRF, copy of valid ID
birth certificate, & other supporting documents to our
frontline services / drop box for processing.
For authorized representative, submit the authorization
letter & copy of valid ID
A. FOR LOCAL EMPLOYEES
Fill out two copies of the PMRF and submit them to HR.
Once your application is processed, PhilHealth will issue
your Member Data Record (MDR) and PhilHealth ID through
your employer.
OR
Submit the duly accomplished PMRF and E2 Form with a
copy of your valid ID and supporting documents like a birth
certificate
proving
your
relationship
with
your
dependent/s.
B. FOR EMPLOYERS
Visit any PhilHealth office and submit the requirements for
employee/kasambahay registration.
After processing, PhilHealth will issue your PhilHealth
Employer Number (PEN) and Certificate of Registration
(which should be displayed in your office, if you’re a
business employer).
Also, receive the MDRs and PhilHealth numbers of your
registered employees or kasambahays.
C. FOR SPONSORED MEMBERS
If your family is listed as NHTS (National Household Targeting System
for Poverty Reduction), get a certification from the city or municipal
DSWD office that you’re an NHTS family.
Submit this document and present a valid ID to the nearest PhilHealth
office.
If your family is not listed as NHTS, go to the barangay office and
inquire about PhilHealth enrollment under the Sponsored Program.

View your account and check membership details like
your address, member category, etc.
Check your premium contribution to ensure all the
payments made by your employer or yourself are
reflected in your account.
Download and print your Member Data Record (MDR)
Pay your monthly contributions (if you’re a self-paying
member).

The MDR contains your basic information, your employer
if you’re employed and the dependent or beneficiaries
that are under your name.
This will prove that you are a member of PhilHealth.
This is also one major requirement when processing for
Philhealth during hospitalization.

WALK-IN APPLICATION
Get a PhilHealth Request Form and fill it out with your
name, PhilHealth number, birth date, and address.
Check the “Member Data Record (MDR)” box.
Submit your accomplished form and present your valid ID
to the PhilHealth officer. Your MDR will be printed and
issued in just a few minutes.

Qualified Dependents
Legitimate spouse who is not a member;
Child or children - legitimate, legitimated, acknowledged and illegitimate (as appearing in birth
certificate) adopted or stepchild or stepchildren below 21 years of age, unmarried and
unemployed.
Children who are twenty-one (21) years old or above but suffering from congenital disability,
either physical or mental, or any disability acquired that renders them totally dependent on the
member for support, as determined by the Corporation;
Foster child as defined in Republic Act 10165 otherwise known as the Foster Care Act of 2012;
Parents who are sixty (60) years old or above, not otherwise an enrolled member, whose monthly
income is below an amount to be determined by PhilHealth in accordance with the guiding
principles set forth in the NHI Act of 2013; and,
Parents with permanent disability regardless of age as determined by PhilHealth, that renders
them totally dependent on the member for subsistence.
Qualified dependents shall be entitled to a separate coverage of up to 45 days per calendar year.
However, the 45 days allowance shall be shared among them.

premium contribution payments are equally shared by the employee

and whose premium contributions are self-paid or subsidized by
another individual through a defined criteria set by the
Corporation.
Note:
For the Formal Sector, the premium will be shared by the employee and the
employer. This will be automatically deducted from the monthly salary.

diagnostic examinations, and drugs and medicines,
initially available to Indigent/Sponsored members,
Organized Groups, and land-based migrant
workermembers and dependents. Recently expanded
to the DepEd personnel. Availed through Rural Health
Units (RHUs) and Out Patient Department (OPD) of
government hospitals.
Z Benefits Benefits designed for illnesses that are
lifethreatening and requires prolonged
hospitalization, extremely expensive therapies or
other treatments that can deplete family’s financial
resources, unless covered by special health insurance
policies.
MDG Benefits and other Special Benefit Packages

of reducing child mortality, improving maternal

How can one avail of PhilHealth benefits?
