Assemble all materials and equipment needed 2 basins Water 6 hand towels Bath towel Clean gloves Thermometer Cotton ball Alcohol swab Patient gown Alcohol Receptacle 4. Perform hand hygiene and observe other appropriate infection prevention procedures such as wearing clean gloves as needed I will now perform hand hygiene to prevent the transfer of microorganisms. 5. Provide for client privacy by drawing the curtains around the bed or closing the door to the room I will now close the curtains around the bed to provide privacy. I will also wear gloves. If it is necessary to let the significant other to enter, allow them. PREPARATION 6. Adjust the bed to a comfortable working height Perform: iikot yung asa dulo para umangat 7. Assist patient to move out of the bed if able Ma’am/Sir, kindly please help your body as I assist you to move out of the bed 8. Place rubber sheet or bed protector on patient’s bed 9. Ma’am/Sir, I will now take your body temperature. I will place the thermometer in your armpit. Prepare the cotton swab with alcohol. We will wait for the beep sound to get the result. Wipe from stem to bulb. Your body temperature is __ degree celsius. 11. Drape the patient with the blanket; remove patient’s gown exposing only the area to be bathed Place the blanket first then remove the patient's gown. 12. Fill the basin with water at appropriate temperature at about 25.9℃ to 32 ℃ I will now fill the basin with water at appropriate temperature at about 25.9℃ to 32 ℃. 13. Using a washcloth, sponge the patient’s face and forehead with plain water only. Do not dry Wet cloth Pat, pat, pat S pattern Place the gown in the hamper. After use, put in the hamper. 14. Place cool, moist washcloth in the axillae and groins. Remove wash cloths when they start to dry 15. Sponge body area as follows: neck, arms, legs, and back. Moisten them for several minutes but do not dry after it is sponged Get washcloth Neck: Pat from the ear to other ear (1side) repeat sa baba Rinse the cloth Both arms: Distal to proximal [from fingertips to the arm] (1side per side) Rinse the cloth Both legs: Proximal to distal [from groin to foot] (1side per side) Rinse the cloth Back: Remove the cloth in the axilla and the groins. 16. Assess patient’s skin, color, or pulse during the procedure Upon assessing the patient’s skin, there are no deviations from normal on its color and pulse during the procedure. 17. Assess body temperature for the first 15 minutes comparing it with baseline data First 15 minutes Wipe from bulb to stem. Ma’am/Sir, I will now assess your body temperature. Continue again for 15 minutes if the desired body temperature had not been reached starting again with item #10 19. Discontinue the tepid sponge bath after 30 minutes. After 30 minutes, the body temperature of the client, it reached the desired body temperature. I will now discontinue the bath. 20. Pat the client dry when bath is complete Pat dry using bath towel 21. Assist the patient to put on a clean gown and change linens if needed, then return the client to a comfortable position Assist the client to wear clean gown AFTER CARE 22. Remove all equipment used away from the bed and place used washcloths, towels, linens, and gown in the hamper Perform 23. Discard used gloves and perform hand hygiene Perform 24. Document the procedure done, patient’s vitals signs, response to treatment, and complications if any I will now document the procedure done including the body temperature of the patient which is ___ ℃, the body temperature of the client decreased and there’s _____ complication observed.
