Description of the problem:

recently
received
a report
on
an
incident which occurred at a Chinese hospital. During an examination, a lithium
battery incorporated in PENTAX Halogen Light Source BS-LH2 exploded and this
caused serious injuries to the user including left-eye cornea burn. No injury to
the patient was reported.
Our investigation revealed that the battery used at this hospital was not the one
specified in the owner's
manual
of BS-LH2.
The batteries specified below are the
only ones confirmed for the safe use with PENTAX Light Sources:

please draw their attention to the risk given and have the batteries replaced by
the correct ones.
Careful attention has to be applied in the use of lithium
