NIAGARA COLLEGE COMMUNICABLE DISEASE SURVEILLANCE INFORMATION SESSION WELCOME TO OUR SESSION! • Communicable Disease Surveillance is an integral part of professional practice in your student placement • Ensuring you have met the requirements protects your personal health as well as the health of the clients you will be working with • The process can take time, starting as soon as possible will ensure requirements will be met prior to the deadline GETTING STARTED HTTPS://WWW.NIAGARACOLLEGE.CA /HWAS/SERVICES/HEALTH/CDS/ Start Here Are you enrolled in one of the following programs? •Autism and Behavioural Science •Educational Assistance and Adult Supports •Early Childhood Education •Gerontology •Personal Support Worker •Personal Support Worker – Alternative Delivery •Recreation Therapy •Recreation Therapy – Fast Track ELECTRONIC REQUIREMENTS VERIFICATION (ESV) • If you are in one of these programs you will be given access to Electronic Requirements Verification (ESV) by Verified, a software platform by Synergy Gateway. • Access will be granted via a secure username and password that will be emailed to your Niagara College email account First Year Students Step 1: Healthcare Provider Letter Download and print the Healthcare Provider Letter to ensure the CDS form is completed correctly. Download Healthcare Provider Letter Step 2: ERV Package Download and print the ERV Package. Download ERV Package Step 3: See Your Healthcare Provider In preparation for your ERV review appointment (see Step 4), please see your Physician at your earliest convenience. Bring a printed copy of your Healthcare Provider Letter and ERV Package with CDS form with you to your appointment. Your physician should fill out the CDS form according to the instructions in the Healthcare Provider Letter. Have all of your forms completed and supporting documentation obtained in preparation for your ERV Review Appointment. Step 4: ERV Review Appointment You will receive an email – sent to your Niagara College email account – from Verified with a link to information about how to book an appointment for your Electronic Requirements Verification (ERV) review, along with information about how to upload your documents. The Verified service will review the documents that you upload and provide a status indicating your eligibility for placement. HOW TO OBTAIN VACCINATION RECORD Contact Public Health Department for region where you attended Elementary or Secondary School. Please note – some Health Departments can take several weeks to provide you with this document Your Family Dr. or Family Health team Your parents or guardians PLEASE NOTE • Please be aware that some clinics will charge for completion of these forms and the entire process can take up to a few months to complete • Once you are cleared you will receive an expiry date associated with your clearance. Some programs require annual TB skin testing and typically this update is all that is required to extend the clearance. There is no need to complete the full CDS form again.
