Dear Mayor Bagasao:
This is in response to your letter dated October 6, 2023, in which you expressed concern on the
update about the resolution (Administrative Order No. 2018-001) regarding the logo and
licensing issues. In this regard, we'd like to let you know that the issue has already been
addressed. Dr. Anthony Cortez, Nueva Vizcaya's Provincial Health Officer, and Ma'am Elizabeth
V. Limidao, Punong Barangay, District IV, Bayombong, Nueva Vizcaya, spearheaded the
actions. In their letter dated September 19, 2023, which appears as follows: “This is in reference
to your letter dated September 8, 2023. We wish to inform that we made necessary corrections to
comply with the provisions of Administrative Order No. 2018-00. Attached hereto for your
reference are the picture of the ambulance showing our compliance. Further, our application for
the license to operate is on process…. we beg the indulgence of your honorable office to allow us
additional period to secure the license to operate since we are just waiting for its approval”
In this letter, it is stated that necessary corrections have been made to address the issue in
accordance with the provisions of Administrative Order (AO) No. 2018-0001 "Revise Rules
and Regulation Governing the Licensure of Ambulances and Ambulance Service Provider"
under Section VI (D), as well as the licensing of the ambulances.
Our office is overjoyed with our stakeholder's immediate compliance, as they adhered to the said
stipulation and thus complied hereto as soon as they received the issue regarding its noncompliance to our order, and with their immediate action and compliance, they were able to
address the issues and comply accordingly.
In this regard, please find attached letter dated September 19, 2023, which was sent to our
office, as well as the corresponding attachments or annexes of photos of the ambulances and the
licensing application form.
We hope that you find this informative and was able to clarify you of the concerns. Nonetheless
this is for the betterment of our offices for transparency and as a reminder to us to always adhere
to what has been stipulated as we uphold commitment to our duties and responsibilities to the
best of our ability, with integrity and with utmost civility.
