I-20 Form Checklist
Students who are accepted into Nyack College programs must also show proof of ability to
cover all academic related costs, and may include in the record of resources any aid offered to
them by the school.

Estimated Budget Worksheet
Print your name and indicate the preferred degree program
of study. This worksheet helps to estimate the total cost
for one school year, and helpful in the affidavit process.

International Student Questionnaire
Provide accurate information on the International Student
Questionnaire; details on this form are needed to fulfill the
information required on your I-20

Affidavit of Financial Support

Have your personal sponsor complete the Affidavit of
Monetary Support. This is required from an individual or
agency stating in $US monetary commitments that must
cover the total cost of attendance for the first year of
studies.

Affidavit of Room and Board

For students with formal arrangements to live off-campus - It
must be filled out by the individual who will be sponsoring
your housing for the duration of your attendance at Nyack
College, Seminary, and Graduate Schools.

Bank Statement or Bank Letter

Provide a bank letter or statement that includes: account
owner’s name, bank stamp and letterhead, date (must be
issued within 3 months from when you confirm), and
showing the minimum funding amount required for I-20
purposes based on your program of study.
Passport Copy
Provide a copy of your passport information page, valid for
at least six months prior to your expected travel to U.S.
US Visa
If you are or have been in the United States, submit a copy of
your current US visa
I-94 Record
Submit a copy of your I-94. You can access the

Current I-20
If you are in the United States on F1 status, submit a copy of your
current I-20
I-20 Transfer Certification Form
Required for F-1 Visa students transferring directly from another
SEVIS program or from current Optional Practical Training (OPT).
