The following are the minimum requirements:
The MVO declaration shall contain/include the following:

MISSION
GOALS
notes:
1) goals must be written in “SMART”
S = Specific
M = Measurable
A = Attainable
R = Realistic/ Relevant
T = Time Bound
2) at least 3 goals per area.
3) the following are the 6 areas: do not change the order of the goals
a. CAREER and /or BUSINESS Goals
b. FINANCIAL Goals
c. FAMILY (parents & siblings) Goals
d. HEALTH Goals
e. RELATIONSHIP Goals
f. COMMUNITY & CHARITY Goals
