Ensure that operator is competent and use signalman/flagman to guide his operation Comply with site speed limit especially in the area of operation, mobilizing to site Equipment shall be inspected and certified by 3rd party before use Carry out pre-use inspection of equipment on daily basis Flagman shall wear hi-viz vest for visibility around equipment to guide equipment during operation at site. All workers shall wear hi-viz vest. No other worker should be working near the equipment during operation Operators must use horn during movement round the site Ensure all rotating parts have adequate guards/ covers Fire extinguisher must be maintained in the equipment during use. Ensure spark arrestor of equipment are in good order and fixed all the time. Working Area to be barricaded off and unauthorized persons kept away. Develop a lift plan and ensure full compliance with lifting plan. lifting equipment in use must be inspected by and confirmed certified by third party certification. No setup of equipment on or in UG barricaded area Using outer rigger pads and Lift supervisor to visually inspect all equipment, wooden pads to ensure they are free from defects. Ensure the ground is suitable to take load. Use web sling to lift rebar cage during welding to insulate and prevent conduction of electricity Use only certified lifting gears. Inspect and confirm that they are in good condition before use. Certified riggers to perform rigging operation Use all mandatory PPEs. 3|Page RISK ASSESSMENT FOR HDD CROSSING WORKS PTW № (if required) Work Title HDD Crossing Works Impact • • • • • • • • • • • • 6. Ensure boom crane operator, rigger and banksman have clear communication (hand signals). Banksman should remain in communication and visual contact with the Boom truck at all times and use high visible reflective vest. Only lift in good weather – no lifting above 9.8m/s wind speed All load shall be tagged and guided by rigger As much as possible avoid lifting over live facilities. Check also for any overhead power lines or other structures Lifting operations must stop if visual contact is lost. These operations can only recommence when a clear line of vision is re-established except a blind lift is planned with perfect communication by radio between rigger/banksman and operator. Ensure the movements direction is away from the personnel live system and facility. Casing and funnel lifting hooks welding and bentonite mixer lifting hooks shall be inspected and certified by 3rd party Works area should be cleared of all obstruction and trip hazards Use assisted lifting for light weights to prevent poor manual lifting.
