As her parent, I hereby accept and assume any and all responsibility for her
welfare, including all financial expenses related to her travel and stay in Japan;

I guarantee that she will abide by all laws and regulations of the Japanese
government;

This affidavit is for the purpose of attesting to the truth of the foregoing facts,
in support of my daughter’s application for a Japanese Visa.
