Training for safety usage of Forklift Maintenance Platform (FMP) for warehouse storekeepers
on 18/8/2023.
Topics briefed to storekeepers:
1. Intentions and purposes where FMP can be used.
FMP should only be used for stock-taking, picking, and put-away of goods and
materials at height, including upper level of warehouse storage racks and roof of
warehouse office/maintenance room; and for any maintenance purposes where ladder
and similar equipment are less suitable and practical.
2. Authorized person to use the FMP.
The FMP should only be used by warehouse storekeeper and maintenance personnel.
Any production operators and everyone else who had no training and experience of
working at height are not allowed.
Supervision and guidance for new and first time-user is mandatory.
3. Identification
A sign is attached to the FMP displaying that only the Toyota Forklift (FL-41) 628FD30 owned by Power Plug Busduct Sdn. Bhd. can be specifically used with the FMP.
It is not encouraged to use the FMP on other similar vehicles that are not identified on
the sign.
4. Safety hazards when using FMP
When inside the FMP, user may be at risk of:

contact with electrical power lines

contact with unguarded moving parts of the forklift

contact with obstructions such as shelving or racks

being unstable in hazardous weather

falling off due to not following safety procedure

being injured if the forklift driver moves the vehicle suddenly
5. Anatomy and purpose of certain features on FMP.

The screen on the back of FMP is to separate people carried from any trapping,
crushing or shearing points on the truck lifting mechanism or any other dangerous
moving parts of the mast.

The integrated locking device behind the heels on the FMP is used to secure itself
to the fork arms.

The floor is slip resistant and designed to prevent accumulation of liquid.

The toe-board prevents any items, materials and tools from accidentally rolling or
dropping out from the FMP.

The gates on both sides can be opened inwards and outwards to provide working
flexibility.
6. Capacity of FMP
Maximum capacity of FMP is 300kg. Only a maximum of 2 persons are allowed in the
FMP at a time and they are not allowed to accommodate additional bulk and heavy
materials or goods inside the FMP in this case.
If goods are to be picked or put-away from and on a warehouse rack, only 1 person is
allowed to be accompanied by the goods inside the FMP at a given time.
If there needs to be 2 persons working at an elevated working space for the picking and
putting-away of goods, then at least 1 person must exit the FMP after being brought to
the said elevated working space in order to avoid exceeding the weight limit from
accommodating the goods along with another person inside the FMP.
The weight of the platform together with its load of people, tools, materials etc should
be not more than half of the actual capacity of the forklift (ie actual capacity for
materials handling).
7. SOPs

The forklift/FMP combination should only be used on firm and level surfaces.

Locking mechanism on the FMP, which includes both gates attached to the front rail
and both levers/chains located behind the heels on the FMP must be inspected before
using to ensure proper functions.

Fork arms on the forklift must be fully extended into the fork pockets underneath the
FMP.

The FMP must be secured to the fork arms via the locking mechanism.

Any overhead obstacles should be determined before initiating the lifting process.
Avoid working in these areas. Switch the position and orientation of working where
there is adequate clearance.

Always perform a trial lift, and make sure the person being lifted can safely reach
whatever they're working on.

The forklift should not be moved while the platform is elevated; however, minor
controlled positional adjustments are acceptable if they are necessary to allow the task
to be carried out safety.

The parking brake must be applied whenever the working platform is elevated.

The transmission must be placed in neutral before elevating the platform.

The gates must be closed and locked with the pin at all time when the platform is being
lifted or lowered.
8. Communications
Forklift driver and the people on the FMP must maintain a constant communication
throughout the use of FMP, especially when raising and lowering. Verbal
communication is preferred since visibility of each other might be impaired when the
platform is raised. Preferably, a third authorized personnel should be present, especially
when surrounding noise level is excessive. He should assist the operation in a safe
manner and help relay information between the two.
9. User of FMP

Make sure that the forklift is on a firm flat surface before entering.

Pre-use checks must be carried out to ensure that the working platform is properly
located and secured to the truck each time and before it is used.

Always enter the cage only from floor level.

Never stand on the guard rails or foot rails.

Avoid leaning out of the platform when it is raised. The platform should be
positioned to prevent the need for people to lean out when carrying out their work.

Do not use stand or climb on anything else while inside the FMP in order to reach
higher.

Keep the cage free from clutter, only have necessary items.

Beware of overhead obstacles to avoid injury and causing damage by knocking,
hitting and denting the FMP.

After using, make sure the FMP is stored in its designated location that is not
exposed to direct sunlight, water, chemicals and harmful substances that can
damage the coating.
10. Segregation of work
The area below the elevated FMP must be kept free from the normal work activity. It is
the duty of the forklift driver or other authorized personnel available to prevent people
and vehicle from passing underneath it. If necessary, other adjacent operations or
activities may have to be stopped temporarily.
