I have attached here evidences of my participation in different
collegial discussions for each quarter of the school year. I have made
Reflection Notes on each collegial discussion that I attended showing
how I planned to enrich my teaching practices from what I have
learned on each activities. With this, I got score of 5 in my IPCRF
Rating for the Quality in this objective. I was also able to attach 4
MOVs, one for each quarter, which gave me a score of 5 for the
Timeliness.

I have attached here evidences of my monitoring and evaluation
of my students’ progress and achievement. Because I have included
an attachment wherein I was able to involved learners in monitoring
and evaluation of their own progress, I got score of 5 for the
QUALITY. For this objective I was able to attach one MOV for each of
the 4 quarters which gave me a score of 5 for the TIMELINESS

To prove that I have sustained engagement through
regular communication of my learner’s needs, progress
and achievement to key stakeholders, I have attached
here Minutes of our Quarterly PTA Meeting with
attached Attendance Sheet and this gives me 5 points in
the QUALITY for this objective. I also got 5 points for the
TIMELINESS because I was able to attach 1 MOV for
each of the 4 quarters in the school year.

For this objective, I have attached examples of my lesson plans
and a community work where I was able to make a reflection of.
In the reflection / journal entry, I was able to detail the consistent
involvement of my students to the teaching and learning process
as a result of applying my personal philosophy in teaching
which is learner-centered. This gave me a score of 5 for the
QUALITY of my submitted MOVs. I also got 5 points for the
TIMELINESS, because I was able to attach 4 MOVs, one for each
of the 4 quarters.
