AFFIDAVIT OF UNDERTAKING
I, __________________________, Filipino, of legal age, with principal address at
________________________________, after being sworn to in accordance with law, depose and state:
1. That I am a Juris Doctor graduate of the University of Makati School of Law (“UMak SoL”);
2. That I received Full Scholarship for the Academic Year 2017-2021;
3. That I am willing to render Return of Service (ROS) for a total of one thousand two hundred
sixty (1,260) hours for a period of three years to UMak SoL and to the City of Makati;
4. That the activities of the ROS shall be defined and coordinated with the UMak Center for
Legal Aid Services (“CLELAS”), which shall include but not be limited to the following:
a. Conduct of traditional and developmental legal aid, as well as other projects connected
therewith;
b. Legal Assistance in the form of legal counselling, rendering of legal opinion,
preparation of affidavits, pleadings, and other legal documents;
c. Represent Legal aid clients before judicial, quasi-judicial and administrative bodies.
d. Perform other services as may be requested by the City of Makati, the University of
Makati and the School of Law;
e. And such other activities that may need the services of a lawyer.
5. That the completion of the required number of hours shall be monitored by the CLELAS;
6. That I am executing this Affidavit to attest to the truth of the foregoing and for whatever legal
intents or purposes it may serve.
