Also block receptors for norepinephrine, histamine, and acetylcholine. Therapeutic Use Relief of psychotic manifestations in bipolar disorder Complications Metabolic sndrome, orthostatic hypotension, anticholinergic effects, urinary hesitancy or retention, dry mouth. agitation, dizziness, sedation, sleep disruption, mild EPS (i.e tremors), elevated prolactin levels, and sexual dysfunction. Contraindications/Precautions Should not be used for clients who have dementia. Use cautiously in clients who have cardiovascular disease, seizures, or DM. Avoid concurrent use of alcohol. Interactions Immunosuppressive medication can further suppress immune function. Additive CNS depressant effects can occur with concurrent use of alcohol, opioids, antihistamines, and other CNS depressants. Medication Administration Oral or IM route. Oral disintegrating tablets, short acting injectable, and extended release injection Nursing Interventions Obtain baseline fasting BG and monitor periodically. Monitor cholesterol, triglycerides, and BG if weight gain is >14kg. Monitor BP & HR for orthostatic changes. Use AIMS test to screen for EPS. Obtain prolactin levels. Client Education Evaluation of Medication Effectiveness Improvement and prevention of acute psychotic manifestations, anxiety, and hostility. Improvement in abilitity to perform ADLS, interact socially, and sleeping and eating habits. ACTIVE LEARNING TEMPLATES Follow a healthy low-cal diet, engage in regular exercise, and monitor weight.
