Work Team Leader and Permit User can be filled by the same person Do it Safely Or not at all … There is always time to do it right ! 3 Permit To Work – User/Approver (JOSOP 400) Area Controller Responsibilities • The Area Controller shall: ― Ensure that equipment to be worked on is released as per approved procedures. ― Ensure that safety controls are in place as appropriate. ― Assist with hazard identification and control measures associated with the proposed job. ― Assist with control measure implementation (e.g., isolation of hazardous energy) as appropriate. ― Verify all specified permit precautions/control measures are into effect. ― Periodically monitor the work site to confirm that the work area is safe, and all permit precautions are being observed. ― Provide notification to Permit Approvers/Work Team Leaders if conditions have changed in their area of responsibility that impacts ongoing work. Do it Safely Or not at all … There is always time to do it right ! 4 Permit To Work – User/Approver (JOSOP 400) Permit Approver Responsibilities • The Permit Approver shall ensure that: ― The permit to work and specialized work permits are issued/approved to the Work Team Leader. ― They liaise closely with other supervisors or persons working in the area. ― The hazard analysis was performed, and potential hazards associated with the proposed work have been identified and suitably assessed in the planning/preparation phase. ― All steps necessary to ensure the safety of the site or installation have been identified. ― The work site has been examined and all precautions specified, including isolations to be taken before work commences, have in fact been taken and shall remain effective while the permit remains in force. ― The Work Team Leader of the work being done under the permit has completed a JSA and is aware of the precautions taken, any additional ones which are to be taken, particular equipment to be used or worn, procedures to be followed, and ensures that this information is effectively communicated to all workers covered by the permit prior to beginning work. Do it Safely Or not at all … There is always time to do it right ! 5 Permit To Work – User/Approver (JOSOP 400) Permit Approver Responsibilities (Continued) ― ― ― ― ― ― ― ― Personnel assigned to specific duties have received necessary training and are competent to carry out these assignments. Work activities that may interact or affect one another are clearly identified, and either conflict is avoided or precautions are included on the permit (e.g., use of welding shields, gas detection, barricades). Copies of all issued permits are displayed at an appropriate location and in a consistent arrangement, so that site personnel can readily see and check which equipment is under maintenance and not available for operation. People are aware of the permit’s duration and of the action to be taken if the work is suspended. The work site is examined periodically while normal work activities are underway, before work is restarted following a work suspension, and finally when the work is completed to ensure that it is in a safe condition. The shift hand-over procedure is properly followed. Any precautions and isolations still to be withdrawn are in fact withdrawn unless they are cross-referenced to other permit activity. The person in charge of the equipment has acknowledged the return of the plant or equipment to their full control. Do it Safely Or not at all … There is always time to do it right ! 6 Permit To Work – User/Approver (JOSOP 400) Site Checker Responsibilities • The Site Checker shall: ― Ensure that all applicable permits have been approved and issued. ― Conduct physical inspection of the work site to ensure that all specified permit precautions/control measures have been put into effect. ― Confirm that a JSA and pre-job safety meeting are conducted with all Permit Users covered by the permit. ― Periodically monitor work site to ensure permit requirements are being adhered to, and that the scope of the job identified on the permit has not changed. ― Conduct a post-job walk-through inspection of the work site to confirm the work is complete, all tools, equipment and isolations have been removed, and the work site is ready to be released back to the facility owner. Do it Safely Or not at all … There is always time to do it right ! 7 Permit To Work – User/Approver (JOSOP 400) Other Roles & Responsibilities ― Personnel involved in permit required activities shall be trained and competent in the roles for which they are responsible. ― A Qualified Gas Tester must conduct gas testing prior to hot work/confined space entry where there is a potential for flammable gases/exposure to hazardous atmosphere is present. The Qualified Gas Tester shall determine the frequency of gas testing based on the potential risk. ― A trained Fire Watch must be present where open flame hot work is carried out, except in designated safe welding areas. ― A trained Entry watch must be present for confined space entries.
