Background The Office of the National Rail Safety Regulator (ONRSR) conducted a compliance inspection on the Sandringham corridor on the 30th of May 2019. ONRSR Inspection Findings During the inspection the ONRSR team spoke to the WGS and TFPC’s at various locations to verify that they were conducting their roles and accountabilities in accordance with “Planning Work Site Protection in the Rail Corridor” procedure L1-SQE-PRO-054 (Version 6). Whilst WGS and TFPC roles and accountabilities are clearly defined within MTM’s procedure several areas of non-compliance were found: The RSWHA completed by the TFPC did not record any controls for the management and movement or RRV’s. The RSWHA completed by the TFPC did not record the Position of Safety for the work group on the protection diagram. The RSWHA that was used by the TFPC was from the previous days shift and was did not reflect the risks and controls that pertained to the activities being completed on that shift. WGS visited by the inspection team was unable to provide evidence that they had recorded and communicated effective risk controls for the movement of RRV's within the worksites. WGS visited by the inspection team was unable to verify the registration status applicable to the RRVs under their control through the use of the PlantGuard App. WGS visited by the inspection team was unable to demonstrate knowledge of their roles and accountabilities for the supervision of worksites in accordance with MTM's Worksite Protection Procedures. MTM was unable to provide that SWMS were contained in two of the RRV's inspected. The RSWHA must identify the Position of Safety for the worksite protection personnel, RSW’s and equipment, and be clearly communicated during the rail safety briefing. The RSWHA must record all controls for the management and movement of RRV’s. Immediate Actions WGS All WGS to have access to the PlantGUARD App. AND be able to verify the registration status of RRVs. All WGS to ensure RRVs are reflected in relevant SWMS. All WGS to record and effectively communicate the risk controls for the movement of RRV's within the worksite. All WGS are to be able to demonstrate knowledge of their roles and accountabilities for the supervision of worksites in accordance with MTM's Worksite Protection Procedures.
