I. REQUIREMENTS OF THE PROGRAM:
A. List of Documents for the student internship
All documents required by the Higher Education Institution (HEI) and Host Training Establishment (THE)
must be accomplished by the student interns before, during and after the internship program.
A.1 Requirements before leaving the university are the following:






Certificate of Registration (COR) as proof that student is officially enrolled in the internship program of the
university;
Medical Certificate issued by the Department of Health (DOH) accredited clinic or hospital;
Notarized Parent Consent;
Approved Memorandum of Agreement (MOA) between HEI and the HTE;
Internship Form 1 (Endorsement Letter) signed by the dean of the college; and
Notarized Internship Form 2 (Internship Contract/Agreement) signed by the student intern, parent/guardian,
internship coordinator, dean, and immediate supervisor.
A.2 During the internship program, the student interns must accomplish the following:


Internship Form 3 (Daily Attendance Sheet) signed by the immediate job supervisor; and
Internship Form 4 (Weekly Status Report) signed by the student intern and the immediate job
supervisor.
A.3 After the internship program the student interns is required to submit the following:



Certificate of Completion issued by the HTE;
Internship Form 5 (Final Review and Evaluation) duly accomplished and signed by the student intern;
and
Internship Form 6 (Performance Evaluation Form) duly accomplished and signed by the immediate
supervisor or head of office.
In the case of the student intern did not complete the internship program from the timeframe set
by the HEI, the student intern must accomplished Internship Form 7 (Internship Request for
Extension) stating the purpose of the said request, to be approved by the OJT adviser and the
college dean.
Orientation and Briefing
After the application period, orientation and pre-internship seminars shall be set. Training expectations,
guidelines, rules and regulations, course requirements shall be discussed. An open forum provides a venue for
clarification on certain issues. The speakers from the Host Training Establishment (HTE) may be invited to share
insights, expectations and experience.
Internship Contract
An internship contract is signed by the student intern, the Higher Education Institution (HEI) and the Host Training
Establishment (HTE). This agreement identifies the student intern’s tasks and some policies regarding the program.
Internship Plan Matrix
A general tasks set by the HEI has to be performed. This task helps the student interns meet the required
learning by the HEI.
Certification
A certificate of Completion with logo of the HTE shall be issued to the student intern after the
completion of the designated number of hours. The certification bears the seal, number of hours
completed, and signed by the HTE’s authorized representative.
