Scenario
: At about 1600H on October 6, 2022, Warehouse Personnel is about to
get the Rodlex C-321 (Degreaser) requested by RCM Personnel. Upon
securing the degreaser, the container accidentally slipped out on his
hand and the cap was broken due to the fall. About 10 Liters of Rodlex
C-321 spilled on the floor.
Incident Level: Level 1 Loss of containment within the process area.
(Reference: 1AP-IMT-001-G001)
Four (4) APRI Warehouse Personnel are involved in the drill and one (1)
Security personnel assigned in the Warehouse.
Emergency spill response materials will be replenished a er the drill.
The spilled degreaser will be a food-grade colored water. All COVID
protocols and guidelines will be observed during the drill.

1. Posi ve: WH Personnel (Hermin Enriquez) triggered the alarm to inform that there is an
emergency in the Area. Manilyn Malagno and the WH team evacuated the area and
assembled.
2. Not familiar with the content of the Emergency Spill Response Cabinet located in the
Warehouse Chemical Area. They (Hermin Enriquez - ) are not that knowledgeable about what
items in the cabinet were gonna be used for containing the spill. (Post Drill - Conduct a
familiariza on of the Emergency Spill Response Cabinet)
