- PSA/NSO Issued COLB

Important Notes:
Without COLB, Learner is not to be
considered as Temporary Enrolled (DO 3, 2018) &
Adviser is not also considered as having incomplete supporting
document in SFCR (DO 11, 2018)

Important Notes: SF 10 is not required during enrollment and not a pre-requisite
for encoding in LIS (DO 3, 2018). Learner’s enrollment must be recorded in LIS in
order to notify the originating school of the transfer. (DO 54, 2016)

 No School Logo and Branding
 Ordinary Bond Paper (hard/specialized paper is not necessary – no need of
procurement for this purpose)
 Print additional Page/s if needed (back to back printout or separate pages
may do, whatever is appropriate and available resources in school)
 1 Grading Box per School Year/School
 if transferred Out, do not make notation (no entries) in the vacant/unused

Important Notes: SF 10 must be prepared at the End of School Year, it should
not be prepared quarterly – (DO 58, 2017 & DO 11, 2018)

The system notification of transfer in LIS is enough for the originating school to
contact the receiving school and provide necessary information for transfer of
document – DO 54, 2016 & DO 58, 2017

 LIS Generated
 Public Schools (except SHS) offering basic education curriculum for core
learning areas shall encode quarterly grades/learning areas in order to
generate LIS-SF5 report. The General Average will be automatically
computed together with the appropriate remark or status.
 For SHS, private schools and other Public Schools that offering
specialized curriculum or program are not required to encode quarterly
grades in LIS. Updating of SF5 shall be done thru encoding of General
Average using the LIS EoSY Facility
Important Notes: SF 5 authorized signatories are the class advisers and the
School Checking Committee. DCC shall no longer required to put their
signatures in SF5. School Head is liable and accountable for any incorrect entries
in the school forms – DO 4, 2014.

h. Extension Name in LIS and SFs
 Identical First and Last Names of father & son
 Encode the given extension name in the designated input
box in LIS regardless of the order in which it appeared in
CoLB
 LIS generated SFs are not subject for editing outside the
system. Format and order of learner’s names are final and
officials.
 The system determine the margins, font type/size and
placement of extension name if any
Important Notes: Encoding of extension name in the designated input box shall
ensure easy retrieval and tracking of learner’s record in LIS database. The use of
an extension name is to identify the father and son if the first and last names are
identical – Civil Code of the Philippines Article 375

2. Report Preparation
a.
DCC deputized checkers shall Accomplish School
Form Checking Report 1B (3 copies, 1 SCC, 1 PSDS,
1 DCC) and fix their signatures

Scores from raffled sections shall become the official

School Forms Guiding Principle -Simplicity
Paano ba dapat I prepare ang mga SFs, computerized/printed or hand written – sulat kamay?
Napansin ba natin kung ano ang guiding principle nang
gawin ang mga SFs? Simplicity – huwag nang pahirapan pa
ang mga School Personnel.
Dahil hindi magkakapareho ang kalagayan ng bawat school, hindi
makatuwiran na magtakda ng iisang guideline para sa lahat, dapat I
consider ang resources at kakayahan ng mga schools. Halimbawa,
pagpapakita ba ng kind consideration kung ipapa computer natin sa isang
school ang mga SFS kung wala naman silang electric or power supply? Or
ipapasulat ba natin ang mga SFs sa isang school na mayroon naman
computer at printer at may manpower skills? Dito na ngayon pumapasok
yung tinatawag na practicality.
Pero siyempre, dapat pa ring I balanced yung “accuracy” – iyan pa din ang
pinakamahalaga sa reporting. Kaya naman sa SF10, sa original nag
uupdate – dahil ito ang ni rereview sa checking, hindi ang electronically
stored data..not yet. - Deped Order 11, 2018.
Kung grade 1, in most schools, practical na gawin ang SF10 using
worksheet format –encoding yan. Pag Grades 2-6, kung may skill naman
para ipasok sa grading box ang mga grades sa pag print – pwede, basta
kaya at di masisira ang original SF10. Pero baka mas madali sa iba ang
isulat ito ng manual.
Kaya kapag nag decide tayo ng approach sa pag prepare ng SFs, in
harmony kaya ito ng guiding principle nang mga SFs- “simplicity”?

Para sa SY2022-2023, Kailan magsisisumula ang Checking of School Forms at the School
Level (School Level Committee)?
Sinasabi ng Deped Order 11, 2018 (sana nabasa na ito ng Checkers mong Magaling) na “ Pagkatapos na
proseso ang grades ng 4th Quarter Examination at mailagay na sa mga School Forms, dapat simulan na
ang checking ng SCC. So kailan yun? Ayon sa Deped Order 34, 2022, ito ay pagkatapos ng June 23
para sa grades 6 & 12 at June 30 para sa iba pang grade levels.

Sa Division Checking Committee (DCC) level, LAHAT ba ng classes/sections ng moving
up/graduating (Kinder, Grade 6, Grade 10 & 12) ay dapat I check ng checkers?
Sinasabi ng Deped Order 11, 2018 na magkakaroon lamang ng Drawlots/raffle para sa mga
sections/classes na I check – hindi kailangan na I check ang lahat ng sections maliban kung ang size
ng classes ay maliit lang (1 section/grade level). Hindi pananagutan ng checkers na I check ang lahat
ng sections dahil ang School Head (hindi ang checkers) ang tanging may pananagutan sa lahat
ng School Forms- Deped Order 4, 2014.

Policy Tidbits
Kung tayo po ay Division Checking Committee (DCC) deputized checker, bakit mahalaga na dapat alam na alam natin
ang focus/concentration ng checking na sinasabi ng Deped Order 11, 2018?
Simula ng gamitin na ang LIS bilang official and only source of
truth ng Learner’s Data and Enrollment sa DepEd, ang LIS data
na ang naging focus ng checking of school forms. Dahil sa
accuracy at integrity na dapat ma maintain sa LIS information,
kailangan ng tulong ng mga teachers, school heads, planning
officers at siyempre, ng ating Division Checking Committee or
DCC.
Ang checking sa “modern time” gamit ang advancement ng
Information Technology ay nag re required sa mga checkers na
alamin ang data na mayroon sa LIS at kung paano it ma
check/validate ng tama para ma ensure ang accuracy.
Kaya kahit matagal na tayong checkers, dapat na nating I
upgrade ang ating knowledge, style at objective sa ng checking –
dahil ang mga teachers na nag papa check ng kanilang forms ay
alam na alam na rin ang updated guidelines ng checking of
school forms – DO 11, 2018

Paano ba dapat I record ang General Average, whole number, round off, round
up?
Sa Senior HS, paano dapat icompute ang Gen. Ave. para ma consider ang
Honors, combined average ba ito ng grades mula Grade 11 & Grade 12 or
combination lang ng 1st & 2nd Semester per Grade level?
Nagbigay ng instruction ang DepEd thru DO 8, 2015 para sa Grading System at DO
36, 2016 para naman sa Guidelines ng Awards and Recognition.
Para tulungan ang mga teachers sa pag compute sa mga grades, ginawa ng mga
volunteer Teachers/IT and Curriculum experts ang Electronic Class Record Template
at inilabas ito thru DepEd Memorandum 60, 2015. Dahil sa ginawa ang ECR sa ilalim
ng guidance ng Central Office, matitiyak natin na ang mga formula at computation na
ginamit dito ay in accordance with the policies.

Paano ba nilalagay ang name ng learner sa Certificate at Diploma?
Siyempre, tama naman na kung ano ang nasa Birth Certificate ay iyon mismo ang
inilalagay sa mga document pero dapat din natin I consider lalo na ang extension
name dahil walang particular na space/area/data field para sa extension name sa
CoLB data capture form kaya hindi talaga nagiging uniform ang arrangement nito.
Sa paglalagay ng name sa certificate /diploma, may mga prototype or
template na makikita sa DO 31, 2019 na mula naman sa DO 2, 2019.
Napansin mo ba kung ano ang order ng names? First Name, Middle Initial &
Last Name. Saan ilalagay ang extension name if mayroon? Saan ba nilalagay
“karaniwan” or general rule ang extension name ayon sa inyong naobserbahan?
Clue, nagkaroon tayo ng President na mayroong extension name…at ang isa pa
ay incumbent, saan ito nakalagay?

Question from Teachers (QFT013)
Kailan ba talaga dapat mag enroll sa LIS at bakit?
Sinasabi ng policy (Deped Order 35, 2022) na sa 1st week palang ng opening of classes, mag simula na agad
mag enroll sa LIS. Pero bakit may ilang advisers na sinasadyang huwag munang mag enrol? Dahil ito sa
MALING PAGKAUNAWA na magkakaroon daw ng problema sa system kapag ang learner ay tumgil sa pag
aaral, hindi daw ito magandang images or reputation sa kanilang school. Kung ganito ang ating
pangangatuwiran, maari nating pag-isipan kung ano ba talaga ang priority or mas mahalaga sa atin, ang mga
learners ba or mas importante sa atin ang images or reputation ng ating school? Kung sakali mang tumigil ang
learner, ang record na makukuha sa LIS ay
malaking tulong para maunawaan ang totoong antas ng dropped out issue at magawan ng solution. Pero kung
hindi
Ito makukuha sa system, paano natin ito matutugunan? Narito ang ilang mga dahilan kung bakit natin dapat
ienroll kaagad ang learner sa 1st week pa lang nang attendance:
1. Tracking. Ma I reregister sa system ang enrollment,
malaking tulong ito sa originating school kasi updated siya
Kung ano ang nangyari sa former learner nila.
2. History. Nakarecord agad sa system ang mga petsa kaya
Hindi ito magkakaroon ng transfer disputes. Malinis ang
Transfer record at History – pati enrollment count ay updated
3. Kapag nag print ng mga School Forms, nandun na ang
Policy Basis: Deped Order 35, 2022
name ng learner, hindi dapat mag-edit.
Paano kung hindi na nagpakita matapos mag enroll?
I update ang status sa NLS, hindi pa yan dropout pwede
Iupdate sa No Status kapag bumalik or Transferred Out kapag lumipat.

Question from Teachers (QFT014)
Kapag nakagraduate na ang isang SHS learner, ano ang mangyayari sa kaniyang
SF10?
Sinasabi ng policy (Deped Order 69, 2016) na ang original copy ng SF10SHS (Formerly Form 137) ay mananatili sa pag-iingat ng SHS kung saan
nakagraduate ang learner.
Kapag ang learner ay nagnanais na ipagpatuloy ang pag-aaral sa higher
education or college, siya ay maaring mag request ng certified true copy
with school seal and original na pirma ng school head. Ang school to
school transfer ng DO 54, 2016 ay para lamang sa mga schools nang Basic
Education. Kapag humihingi ng Original Copy ng SF10, maaring ipakita
ang DO 69, 2016 na nagbibigay ng instruction sa mga SHS.
Ang holding of document na pinapayagan ng DO 88, 2010 ay para lamang
sa mga private schools na mayroong Basic Education Curriculum. Hindi
maaring gamitin ng Private School ang provision na ito kapag ang
utang/balance ay sa College.
Dapat magbigay ang lahat ng SHS ng certified copy ng SF10 sa
Division Office –Public man or Private.

Hangang Kailan Ba Maaaring Tumangap ng Enrollment sa Public School?
Sinasabi ng policy (Deped Order 35, 2022) para sa SY2022-2023 ang enrollment period ay nagsisimula ng July 25 hangang August
22, 2022. Layunin ng policy na ito na lahat sana ng learners ay nasa school na or nakaready na sa unang araw pa lamang ng klase
dahil bawat araw ng “class days” ay pagkakataon na matuto. Malaking tulong din sa school at class management kung makukuha
na agad sa unang araw pa lamang ng klase ang kabuoang enrollment count para ma iconsider ang mga resources na kakailanganin
at makagawa ng mga decisions para sa kapakinabangan ng mga learners.
Kaya nga lang sa actual na mga pangyayari, hindi lahat ng
learners ay nakakapag enroll sa schedule according sa policy.
Dahil sa ibat-ibang kalagayan, may mga learners na nakakapag
Enroll ilang araw, o mga weeks pa nga after school opening,
tinatawag itong LATE ENROLLMENT. sa Deped Order 3, 2018
possible pa din naman na tumangap ng learner kung mayroon
pang natitirang 80% ng school days. Sa ibang mas kakaibang
mga cases, ang School head ay maaaring magpasiya na tangapin
pa din ang Learners depende sa magiging benefits nito sa learner.
In general, mabuti pa din kung lahat ng learners ay nasa school
na day 1 pa lang, pero sabi nga ng kasabihan, It is better late than never.

Isa akong class adviser at mayroon din akong 6 hours of actual classroom
teaching load (360 minutes/day) , paano po ito dapat mag reflect sa SF7 at Class
Program?
Sinasabi ng policy (Deped Memorandum 291, 2008) na ang isang public school teacher ay hindi exempted sa 8-oras na
pagtatrabaho para sa isang karaniwang government employee gaya ng sinasabi sa R.A. 1880. Kaya naman sa Magna Carta for
Public School Teacher, anim (6) na oras sa isang araw or 360 minutes ang inilalaan para sa actual classroom teaching at ang
natitirang 2 oras naman ay para sa teaching related activities at lesson preparations. Pero pansinin, malinaw na sinasabi ng DM
291, 2008 na kung Ikaw ay isang class adviser, dapat kang magkaroon nang 1 oras Or 60 minutes na equivalency at dapat
ibilang ito na karagdagang 1 teaching load.
Kaya naman, kung ang class adviser ay mayroong 360 minutes
Or 6 hours of actual classroom teaching bawat araw at idaragdag ang 60 minutes equivalency, dapat mag reflect sa Class
Program ay 420 minutes. Sa ganitong computation, lalabas na
ang class adviser na ito na may 420 minutes or 7 oras na total
Teaching load at 2 oras para sa teaching related/lesson
preparation ay may total workload na 9 na oras bawat araw.
Malinaw na ang class adviser ay mayroong 1 hour excess workload/day.

Financial obligation kapag nag coconfirm sa transfer request.
Paano kung kahit napaliwanagan na ang private school tungkol sa sinaSabi ng policy, wala pa ding action or worse, nag declined pa?
Dapat tandaan ng private school na anumang action or kahit I pending
ang transaction ay mayroon itong negative effect sa ibang school. Kaya
Naman, ganito ang dapat gawin ng affected receiving school:

Gumawa ng report sa naging action or sinasadyang pag pending at
ipadala ito sa Schools Division Superintendent (attn: SGOD)
2.
Ang SGOD thru DPO ay mag validate sa report at gagawa ng
letter for signature of SDS para ipadala sa concern private school
3.
Kung nasa ibang ibang Division ang private school, ang letter at
report ay i aaddress sa SDS ng nakakasakop sa private School.

Sinasabi ng policy (Deped Order 35, 2022) na LAHAT (baguhan at datihan) kasama na ang ALS learners ay dapat mag fill up ng Enhanced Basic
Education Enrollment Form (EBEEF) at ito ang magiging basis sa pag record/update sa LIS Enrollment Quick Count. Dahil sa mga bagong data
elements na
na hinihingi, kahit yung mga learners na dati na nag-aral sa school natin
at may record na sa LIS ay dapat pa din mag accomplish ng EBEEF.
Kapag nagging available na ang LIS BOSY facility at kailangan nang magEnroll sa LIS, ang na accomplished na EBEEF ang magiging reference

grade 1?
Sinasabi ng policy (Deped Order 47, 2016 Omnibus Policy on Kindergarten Education) sa rationale nito “mandatory and compulsory”
Kindergarten education…..to all five (5)-year old Filipino children to sufficiently prepare them for Grade One.
Nasa section VI Enrollment Procedure (par.15.D) naman ang mga qualification para sa incoming grade 1 (Eligibility for Grade 1). Ganito ang
instruction para sa sub par.ii “Children who are six years old and above who have not completed Kindergarten due to difficult
circumstances and/or extreme poverty will have to complete the Kindergarten Catch-up Education Program (KCEP). At the end of
the KCEP, teachers will have to assess the learners…” These document will have to be turned over to the
Learner’s Grade 1 Teacher…”
So, mag KCEP muna (enroll to Kinder kasi hindi naman pwede
Na Grade 1 teacher mag provide ng KCEP). Pagkatapos ng KCEP
intervention, need ng assessment para malaman kung ready na
sa Grade 1 (and duration ng KCEP ay depende sa progress ng
Learner. Itanong sa School Head at CID ang iba pang detalye ng
KCEP.). Kaya dapat munang ienroll sa LIS as kinder at tapos
correction of grade level to Grade 1 kung ready na as per ECCD Checklist.

Ano ang dapat gawin kung mayroong transferred in na learner from school
abroad?
Mayroon pong dalawang klase ng school abroad, Philippine Schools Abroad (PSA) at Foreign School Abroad (FSA).
Ang mga PSAs ay accredited ng Deped at mayroon silang School ID kaya naman ang mga learners nila ay mayroon nang
LRN. Ang pag transfer ng mga learner from PSA ay katulad lang ng pag transfer sa mga schools sa Pilipinas. Kailangan pa din
na ienroll muna ng receiving school sa Pilipinas, galling sa PSA (or vice versa) ang transferred in gamit ang Enroll Learner
Facility ng LIS. Magpapadala ito ng system notification sa originating school. Mayroong 30 days para umaksiyon ang
originating school to confirm at ipadala ang SF10 na may kasamang attachment.
Ang FSA naman ay hindi accredited ng Deped kaya wala itong LIS account.
Ang mga learners mula sa FSA na mag-aaral sa Pilipinas ay kailangan munang
dumaan sa school-based assessment ng school upang malaman kung
possible bang ituloy ng learner sa next higher grade level ang natapos
niya mula sa FSA. Kailangan pa din na mag submit ng credential ang learner
mula sa FSA na dating pinag aralan at mag submit ng kopya ng Birth Certificate
I pa translate sa English ang document para maunawaan. Kung ang learner ay
lumipat nang hindi pa tapos ang school year sa FSA, Kailangan ng PEPT.
Para ma isyuhan ng LRN, isubmit sa SDO ang mga Document (BC, Transcript &
School Assessment Report) for reference ng DPO sa approval ng LRN issuance.

Private School sa LIS Transfer Request?
Dapat tandaan ng Private School na ang Transfer Facility ng LIS ay para ma masubaybayan at
ma irecord ang enrollment history ng learner – hindi ito Loan Information System –kaya huwag
sana gamitin sa paniningil. Pero kung ang lahat ng schools ay mag update ng Learner Transfer
Status, malalaman natin ang present school location or enrollment ng learner –tulong ito sa
private school.
Paano kung hindi pa nakakabayad ang learner, dapat bang mag confirm ang Private
School sa Transfer Request?
Sinasabi ng Deped Order 35, 2022 (as reiterated from DO 32, 2021), within 30 calendar days
upon receipt of LIS notice of transfer request, ang originating
Private School ay dapat gumawa ng confirmation kung ang learner ay talaga bang hindi na nagaaral sa kanilang school. Hindi na dapat Hintayin pa ng Private School na makapagbayad
ang learner bago I click ang CONFIRM
button dahil ang confirmation dito ay para lamang tiyakin na ang Learner ay:
1. WALA na sa School namin (For Tracking Purposes)
2. At nang umalis ay may NAIWANG UTANG (With unsettled Account)

Character para makapag-aral sa Public School?
Hindi na po. Noong 2016, inilabas ang Deped Order 54, 2016 Guidelines on
the Request and Transfer of Learner’s School Record, Form 138 (SF9) at
Form 137 (SF10) lang ang kinakailangang mga requirement para sa transfer.
Sa inilibas naman na Enrollment Guidelines noong 2018 (Basic Education
Enrollment Policy), hindi kasali ang Certificate of Good Moral Character sa
“Eligibility and Documentary Requirement” (Sec. V Enrollment Procedure).
Sa mga sumunod na Enrollment Policies in the context of pandemic (DO 8,
2022, DO 32, 2021 & DO 35, 2022), hindi nga man lang nabangit ang CGMC
kaya lalong hindi ito kailangan sa enrollment requirement.
Pero bakit hindi inilagay sa mga Deped Orders na iyan na hindi na
pala kailangan ang CGMC?
May mga information na sa SF9 & SF10 tungkol sa character or personalidad
ng isang learner – at makakatulong ito upang higit pang matulungan ang
learner.
Kaya kung hindi kasama ang isang document tulad ng CGMC, lohikal lang
na isipin na hindi nga ito kailangan. Hindi ba mas madaling unawain at
sundin kung alam natin kung ano ang tama kaysa sa isang mahabang

Kapag ang Learner ay galing sa hindi authorized/accredited na private school,
maituturing ba siyang Temporarily Enrolled?
Sa konteksto ng Deped Order 3, 2018 at sa Annex 3 Affidavit of Undertaking, ang mga Temporary Enrolled ay ang mga Learner
na may naiwang pagkakautang sa legit private school at ito ang dahilan kung bakit hindi ma I transfer ang kanilang credential.
Ang sitwasyong ito ay magkaiba sa kaso ng learner na galing sa isang private school na hindi authorized/accredited dahil unang
una, hindi authorized ang private school na ito nag mag issue ng anumang legal na document dahil hindi naman legal or
authorized ang school. Anumang document ang I release ng private school na ito ay null and void ,hindi valid, walang value/not
credible.
Kaya hindi pwede gamitin ang Annex 3 or Affidavit of Undertaking dahil sa mga
declarations nito sa second condition “With These circumstances,
I undertake to: 1. Do what is legally permissible for the release of the credential
of my child from the previous school. 2. Submit the transfer credential of my
child & 3. ….submission of school credential from the previous school.”.
Kung hindi applicable ang Temporary Enrolled, ano ang solusyon?
Sinasabi din ng DO 3, 2018 sa ilalim ng “Special Cases”: Learner from Non
Deped Accredited School, dapat munang kumuha or mag pa validation/
placement exam sa BEA ng PEPT/PVT at ang resulta nito ang magiging basis
kung anong grade level eligible ang learner. Hindi maaring I enroll sa LIS ang
learner na walang eligibility proof or document tulad ng PEPT/PVT rating dahil
ito ang magiging basis ng Planning Officer to approved ng issuance of LRN.

Bakit Kailangan ang Birth Certificate sa Enrollment?
Noong unang panahon, tinatantiya (estimate) ang edad ng isang bata pamamagitan ng pag-abot
gamit ang kanang kamay (righ hand) na pinadadaan sa ibabaw ng ulo patungo sa kaliwang teynga
(left ear), kaway kaway sa mga batang 70’s at 80’s!
Sa konteksto ng Enrollment, ang Certificate of Live Birth (COLB) ang pangunahing reperensiya upang
patunayan ang pagkakakilanlan o identity ng isang learner. Anumang dokumento ang gagawin ng school para sa learner ay dapat na kasuwato o
tugma sa nakalagay sa COLB. Ito din ang basis sa pag register sa Learner Information System or LIS. Kapag ang LIS record ay hindi nag
matched sa COLB, ang LIS record ang dapat mag adjust hindi ang COLB.
Paano kung ang isubmit ng learner or ang nasa current file ng school ay NSO issuance pa, dapat bang mag submit ng bagong PSA
issuance? Hindi na kailangan.
Kailangan bang original copy ng PSA/NSO ang isubmit? Hindi ito required. Pero anumang COLB ang isubmit, original man or photo copy,
dapat ipaunawa sa magulang na anumang document ang isubmit nila, ito ay magiging basehan ng mga document or form na gagawin ng
school or deped para sa learner, kaya anumang pagkakamali sa COLB ay tiyak na magbibigay ng malaking problema sa document or record
ng learner at sila din (magulang/learner) ang mas maaapektuhan.
Paano naman kung hindi pa naka registered ang learner ng local civil registrar, ano mga document ang maaring isubmit? Kung mayroong
Baptismal Certificate pwede itong gamitin. Ang Barangay Certificate na mayroong basic profile ng learner ay dapat ding tanggapin ng public
school.
Ang learner ay hindi naman mag-aasawa or mag aaply ng work, gusto lang niya makapag-aral kaya hindi dapat maging hadlang para hindi natin
siya tangapin sa Public School kung hindi man siya makapag submit pa ng Authenticated COLB
