Instructions/Reminders to Passers of EPS-TOPIK via Computer-Based Test
(CBT)
Relative to the released names of applicants who passed the recently concluded EPSTOPIK via CBT last November 18 – December 21, 2021, please be guided of the following
procedures/requirements to be strictly observed.
1. Considering the current Covid Alert Level in the NCR, No personal submission
of the required documents will be accepted but only thru the following options:

Via online, thru ebmrd.eps@poea.gov.ph
In your email message, please consider and indicate the following for
Proper and easy monitoring:
➢ Subject: Passer of EPS-TOPIK via CBT 2021
(Last name, Given name and Middle initial)
➢ Message: Full name (Last name, Given name and Middle name)
Birthday:
____________________
Passport Number: _______________
Note: All documents must be consolidated in just one/single file, in PDF
form, per worker

2. Required documents:
2.1
Scanned copy of valid/latest Passport (Information page only)
2.2
Copy of Medical examination result from a DOH accredited hospital/Clinic
with examination conducted not earlier than January 10, 2022 (note: make
sure that the result is also signed by the applicant in the appropriate portion)
3.3
Duly accomplished “Information Sheet for EPS Passers” Form (copy hereto
attached)
In filing-up of form, please be reminded of the following:
a. All required data must be provided. All questions thereat must be
answered properly;
b. College graduate applicants must likewise state the name of High
School where they graduated;
c. Incomplete data provided as submitted may result in the delay in
encoding and inclusion in the jobseeker Roster.
