Details of Observations:
1- RT Films not matching
ROOT CAUSE:
1- Two times marking of zero were observed and repairs were attended two times according to two
times zero marking of RT and up to R2 .
Corrective actions:
1- After finding root cause of issue, we took whole joint,o-o, to observe defect free joint.
2- RT marking shall be on pipe with proper numbering and segment wise as defect can be traced
easily.
3- Defect shall be traced with tracing paper and related QA/QC inspector shall be responsible for
repair marking.
4- NDT agency shall be responsible for proper Location marking on joint.
