PRACTICUM CONTRACT/AGREEMENT

COLLEGES as represented by Ms. Emy C. India,
THIS AGREEMENT is made by and between the TRIMEX
Trojan Building, Brgy. Poblacion, Biñan,
Program Chair, Social Work Department with school address at
Laguna

Ms. Adora C. Pascual, RN,
City Social Welfare and Development Office, Cityof Biñan as represented by
8arangay Zapote City of
RSW, Department Head, with office address at Ground Floor, Annex Building,
Biñan, Laguna.
WITNESSETH:
1. This Agreement shall govern the relationship of the Social Work Practumer and the City Social
Welfare and Development Office, Biñan City where he/she will be assigned and does not create
Department Office and
any employer-employee relationship between City Social Welfare and
Trimex Colleges Interns;
2. That, the duration of this Agreement shall be for a period of five (3) months to be in force and
effect within September 2022 to November 2022.The Practicumers should accumulate a total of
500 working hours within the period. In cases where the Practicumers
will not be able to finish
the specified time, shall inform/notify City Social Welfare and Development Office (CSWDO) and
Trimex Colleges through his/her immediate supervisor for the intention of internship extension;
3. That,this practicum is educationalin nature with an opportunity to complement formal learning
with practical knowledge, skills,and a desirable attitude to gain hands-on experience;
4 That. the City Social Welfare and Development Office, City of Biñan shall guarantee quality
learning and exposure, and safetywhile undergoing practicum and considering the nature of the
program by training the lnterns under actual work conditions:
5 That. the City Social Welfare and Development Office, City of Biñan will issue a final grade to the
practicum student upon completion of requirementsand performance assessment;

That is, the Practicum students receive direct and close supervision by an appropriate supervisor.
The practicum student is not entitled to have allowances but are expected to acquire professional
work ethics and skils to address the demand of the agency;

That, the PracticCum student will maintaina regular schedule determined by the
practicumer and
the supervisor, will demonstrate honesty, punctuality, courtesy,
cooperative attitude, proper
health and grooming habits, appropriate dress and a willingness to learn, will obey the
policies,
rules,and regulations of the agency;

That,the Practicum student will furnish his/her supervisor with all necessary
information, related
assignments, and reports;
9. Confidentiality: The Practicum student must maintain confidentiality, when and where
appropriate, during and after the internship, of all the data and information where such
information is not already within the public domain and
indicated or understood to be
confidential.

Below are the expected hours, reporting in the office, roles, tasks, functions, work details and assignments,
cases handled, and others as needs arise as they are doing their field practicum:
STUDENT:

From September -November they will be able to render a total of 500 hours

Reports to assign office from Friday, Saturday, and Sunday. They can do other tasks that are related to

Atrialogue meeting before and after the field practice

The agency will be the one to assign cases to be handled.
Assign5 cases for group work intervention however 3 cases will be actively handled for case
management and applied different social work interventions like home visits, counselling, and other

socialwork intervention.
Assign the interns that they can experience intake interviews and make their assessment or anv
assignment that they can experience a social work.
Attend meetings and supervision of the agency and schoolsupervision.

Submit all the needed documents like daily timesheet, work plan, social case study report, and other
pertinent documents
10. Come up with a group case study report.
11. Applied the helping process and case management
12. The student report regularly to their agency supervisor
13. Acase presentation and conference should be done in the agency and the final case presentation should
be done in school with a panelof reactors.

Provide supervision like technical, support, and administrative especially in assigning cases.
Monitor the student on the assigned task given to them.

Provide an atmosphere where they can apply the things that they learned from school/classroom.

An evaluation form should be done by the agency's Supervision for their grades.
