Prior to Operating the accelerator, the operators will know the location of emergency switches and Main circuit breakers. "EMERGENCY OFF” buttons are located on the console, on the Modulator, and the walls in the treatment room. The EMERGENCY OFF buttons control both the beam and the motors driving the mechanical movement of the couch and the Gantry. Procedure: A. Treatment Beam Fails to Terminate: 1. Immediately press the BEAM OFF button and turn the DISABLE/ENABLE key switch tothe DISABLE position. 2. If the beam remains on, press the nearest EMERGENCY OFF button. 3. If the beam still remains on, turn off the circuit breaker near the console. 4. Remove the patient from the couch. 5. Check the backup counter and document the cumulative number of monitor units delivered. 6. Notify physics/engineer AND do not attempt to operate the Unique until Physics/engineerreleases the unit for clinical use. 7. Document service call and error in logbook, and daily warm up sheet. Power Failure AND Equipment Malfunction: 1. Terminate the beam if applicable. 2. Remove the patient from the couch. 3. Record the backup counter reading at the time of failure. 4. Deliver the remaining monitor units. 5. Notify physics. C. Electrical Fire: 1. Depress the nearest EMERGENCY OFF button. 2. Turn off the main circuit breaker and contact Maintenance. 3. Remove the patient from the couch. 4. Initiate Safety procedures for f ire during working hours. 5. Call the engineer. 6. Record the entered treatment monitor units. 7. Calculate the remaining monitor units. 8. Notify physics. Accidental Radiation Exposure: In the event a person is accidentally exposed or thought to be exposed to radiation, notify theMedical Physicist or Radiation Safety Officer (RSO) immediately. E. Patient Support Assembly (PSA) Life Mechanism Failure: Should the PSA fail to lower upon command from the pendant control when a patient is onthe PSA, perform the following procedures? 1. Verify that no emergency buttons have accidentally been pushed. They should all be pulledout. 2. Reset motors, if couch will not lower, reboot linac. 3. If the PSA still does not lower, contact engineer. 4. Remove patient by raising stretcher, assisting the patient over to the stretcher, and loweringthe stretcher.
