Scenario
Around 10 am on 100402012 in Uta mania glop 12wellhead 1590 while handing the oil bucket by the
workers , it fell on the ground and spilled. The worker immediately lifted the bucket to stop spreading and
called area supervisor and co workers who area trained ESRT .the co worker bring the spill kit to spill location
and use the spill kit containing and cleaning the spill .the activities foreman inform incident to environment
coordinator. The environment coordinator notified spill to project manager’s manager and SMPT immediately
and reaccessioned sit observe the environment incident .
