THERAPEUTIC USES:
Mild to moderate pain, decreases fever,
reduces
inflammation, musculoskeletal disorders such
as
rheumatoid arthritis and osteoarthritis
ACTION:
Inhibit prostaglandin synthesis by
blocking
cyclooxygenase (COX)
Inhibit pain and also inhibit enzyme that
maintains lining of stomach
ADVERSE EFFECTS:
GI upset - nausea, vomiting, diarrhea,
abdominal
pain, discomfort (most common)
Hypertension and heart failure
Nephrotoxicity
Blood clots - stroke
NURSING INTERVENTIONS:
Infection
Administer proton pump inhibitors such
as
omprazole and pantoprazole for GI upset
and
acid reflux
Avoided during second trimester of
pregnancy pregnancy D category
.
MEDICATION/FOOD/ENVIRONMENT
INTERACTIONS:
Take with food - do not take on an empty
stomach
Decreased diuretic effect when these drugs are
taken with loop diuretics; there is a potential
for
decreased antihypertensive effect of
beta-blockers
if these drugs are combined; and there have
been
reports of lithium toxicity, especially when
combined with ibuprofen
CLIENT EDUCATION:
Instruct pt. to take with a full glass of
water and
remain in an upright position for 15-30
min. after
administration
Teach pt. to avoid alcohol to minimize
gastric
irritation
MEDICATION ADMINISTRATION:
PO - administered after meals or with food,
full
glass of water
Rapidly absorbed from GI tract, metabolized
in
liver and excreted in urine
EVALUATION OF MEDICATION
EFFECTIVENESS:
Relief of mild to moderate pain or
discomfort reduction of fever

SPECIAL CONSIDERATIONS FOR PARTICULAR
DRUGS IN THIS CATEGORY:
Ibuprofen (advil),
fenoprofen (Nalfon),
celecoxib
(celebrex),
naproxen (aleve),
diclofenac,
etodolac,
meloxicam
Antidote: no specific antidote - sodium
bicarbonate (IV) to treat toxicity, activated
charcoal for ibuprofen
Take with food to decrease stomach upset,
do
not take on an empty stomach
Hypersensitivity to aspirin or other
NSAIDS is a
contraindication for whole group of
NSAIDS
