1. Miss Li, a 40-year-old woman, was diagnosed with left Colle’s fracture after a fall injury
with left outstretched hand landed on the floor. Closed reduction and left short arm dynacast
was applied to her. She was planned to discharge home after the correct position was
confirmed by x-ray.
a. Discuss the 5 P’s assessment to Miss Li.

1. Mr. X, a 65-year-old man, was diagnosed with Osteoarthritis of left hip 8 years ago and
diagnosed with gouty arthritis currently. Mr. X has undergone right Total hip replacement in
your ward 6 days ago and is planned to discharge home these few days.

2. Mr. A is a 70 years old man, attended Accident and Emergency Department (AED) because
of an uncontrolled nasal bleeding. Mr. A has a history of hypertension for 10 years and was
diagnosed of atrial fibrillation last year. He is currently on Atenolol 50mg and Aspirin 80mg
daily and deny any nasal trauma prior to the bleeding.

Describe the immediate management to Mr. A.

Mr. A is being transferred to ENT ward for Nasal packing due to the uncontrolled nasal
bleeding.

1.
A 30 yo women, Mrs. Li, was found collapsed in MTR in the early morning with loss of
consciousness (LOC) for 2 mins. Up-rolling eyeballs, shaking of legs, which lasted for 30
sec., were noted before the LOC, witnessed by her husband. Mrs. Li cannot recall her
incident after regain consciousness. She has the history of childhood epilepsy but no longer
required any medication now. She was then admitted to ward for investigation.
a. Name the type of seizure that Mrs. Li experienced.
(1 mark)
b. After reviewing the patient’s history, doctor identified that Mrs. Li has repeated seizure
in the past 6 months and advised her to re-start anti-epileptic's drug, Sodium
Valporate. What education information should be given to the woman if anti-epileptics
drug is advised?

After a series of investigation, Mrs. Li was found with a lesion over the frontal lobe which
suggestive for primary brain tumor.
c. Suggest TWO possible investigations for Mrs. Li and state the possible findings.

d. Mrs. Li was suggested for surgery and followed with radiotherapy. Explain the
indications and the related nursing care after surgery and radiotherapy.

2. Mr. Wong was admitted to the hospital for further investigation and management after his car
was hit from behind by another car which did not slow down before the red traffic light. Mr.
Wong complaint of severe headache over the occipital area, nausea and dizziness. No
obvious head injury or open wound noted. X-ray shows no C-spine injury or fracture.
a. Explain the possible mechanism of the head injury of the driver.

b. CT scan showed subdural hematoma over the posterior fossa. LIST the change of objective
parameters resulted from “Cushing reflex” and TWO more symptoms that indicate the
deterioration of the situation and required immediate treatment?

c. Mr Wong has undergone urgent cranial surgery to control the situation and was then
admitted to Intensive Care Unit for the close monitoring. Describe the monitoring
components and the nursing cares related to ICP control and wound and drainage care.

a. Explain the difference between open-angle vs close-angle glaucoma in terms of mechanism of
intra-ocular pressure maintenance.

undergone laser trabeculoplasty.
