LMTO’s payment term for invoices is Net 45 Days upon receipt of invoice. Certification of proposal validity for 60 days is requested. 2. The following enclosed forms certifications will need to be completed and returned with your proposal response: (a) Supplier Profile (b) Self Certifications per Corpdoc 2 dated July 2002 3. Offerors producing products where the country of origin is outside the U.S. or its territories and possessions must identify the place of production or performance in the offer. 4. Offerors providing products with an origin outside the U.S. or its territories and possessions must identify ozone depleting chemicals used to manufacture the products. 5. Offerors providing products containing ozone depleting chemicals must label the products in accordance with federal law. 6. Offerors not responding with an offer shall return the solicitation materials and must notify Lockheed Martin by fax, email, or phone to remain on LMTO’s source list. 7. Make all contacts relevant to this solicitation with the authorized Subcontract Administrator. Unauthorized contacts are a basis for disqualification. 8. Offerors may cancel offers, or correct errors in their offers, at any time prior to award. 9. Offerors must not disclose any information in the solicitation to anyone who does not have a need to know. 10. Answer completely each element and item of information requested, and explain and justify any omissions. 11. Offerors must state any changes to their business size or ownership characteristics, if any have occurred since their last written certifications were furnished. 12. For any services to be performed on Lockheed Martin or U.S. Government property, the following applies (item 13). 13. In compliance with the attached Supplement C, the selected Subcontractor must provide a valid Certificate of Insurance, certifying to the existence of coverage meeting the minimum requirements stated therein, prior to the start of any Agreement or Subcontract. 14. Identify any data, and data rights that are proprietary. LMTO shall not receive or provide proprietary information without a Non-Disclosure Agreement with subcontractor. 15. The terms and conditions applicable to this solicitation and any resultant contract include self-certification statements (per Corpdoc 2) pertaining to the offeror's debarment status and other matters. In the event offeror cannot affirmatively accept any of the self-certification requirements, offeror's proposal must so state and include any explanation therefore. ADDITIONAL BID INFORMATION 1. This procurement is in support of the Defense Threat Reduction Agency Program, Prime Contract No. DTRA01-02-C0095 and does not have a DPAS Rating.
