RESOLUTION NO.04, SERIES OF 2023
A Resolution of Bulacnin Elementary School Bids and Awards Committee declaring
the Mode of Procurement of office equipment supplies and consumables under PR
No. 23-03-03.
WHEREAS, the BAC secretariat posted the Request for Quotation RFQ) with
Reference Number 9567342 for Office Equipment and Supplies for Consumables
with PR No. 23-03-03 in the PhilGEPS website from March 9, 2023 (First Posting)
and March 15, 2023 (Second Posting) with an Approved Budget of Php111, 900. 00
WHEREAS, in the first posting there was only one bidder who signified the interest
to the school supplies and equipment while on the second posting, there were four
bidders where the previous bidder reposts their intention to comply.
WHEREAS, Bids and Awards Committee Secretariat from the SDO Lipa City posted
a Request for Quotations (RFQs) for the procurement of the above-mentioned school
office equipment and supplies of the four suppliers, namely;

Holy Angel Trading
RR Lim Office and School Supplies Trading
Razel Enterprises
WHEREAS, on different dates prior to the deadline of submission of price quotations,
quotations from four (4) suppliers were obtained by the BAC Chairman through
e-mail, attested by the accomplished Request for Price Quotation (RFQ).
WHEREAS, in the course of the evaluation of the bids by the BAC, the following are
the result:

WHEREAS, the lowest cost bidder is of risk of unavoidable damages due to location
and distance of travel
WHEREAS, on March 21, 2023, after the conduct of the mandatory review of terms,
conditions, specifications, and cost estimates, the approved bidder, OZZY SCHOOL
SUPPLIES was found to be the most advantageous and responsive quotation.
NOW, THEREFORE, We, the members of the Bids and Awards Committee, hereby
RESOLVED:
a. To declare the award of contract for the Procurement of School for Office
Equipment and Supplies for Consumables under PR. No. 23-03-03 dated March 21,
2023 to Ozzy School Supplies in the amount of One Hundred Five Thousand Six
Hundred Seventy Pesos only (Php105, 670.00).
b. To recommend for approval by the School Head of the Bulacnin Elementary
School, Schools Division of Lipa City the foregoing findings.
c. RESOLVED, at Bulacnin Elementary School, this 21st day of March 2023.
